2 Quality Health Indicators  The Quality Health Indicator (QHI) web site was developed through KHA and KRHOP to facilitate a benchmarking project for rural Kansas hospitals.  The goal of QHI is to provide hospitals an economical instrument to evaluate internal processes of care and to seek ways to improve those practices by comparing specific measures of quality with like hospitals.  Using QHI as a tool, regional networks of hospitals can select from a library of quality indicators to determine which measures meet their unique needs.

3 Quality Health Indicators  in 2005, the Multi State Benchmarking Project selected QHi as their data collection tool. The Project involves Critical Access Hospitals in Kansas, Michigan, Montana, Nebraska and South Dakota. (60 hospitals)  Through the Multi State Benchmarking Project, QHi is well positioned to serve as a significant repository of information on quality of care and performance in Critical Access Hospitals nationwide.

8 Enter User Name and Password to log into this secure site User Names and Passwords are Case Sensitive 8

9 A user name and password is required to enter this secure web site. The password must be 8 characters that include upper and lower case letters and at least one number. For example: myQhi789 The level of access is determined by the user type. System Administrator – maintains the site (KHA) State Administrator –provides support to Hospital Contacts in their State Network Administrator – maintains Network profiles & provides support Hospital Contact – maintains Hospital profiles, adds users & enters data Hospital User – enters data and runs reports View Only – views data and runs reports Quality Health Indicators

10 The Welcome Screen provides contact information, data entry timeline and special notices. Special alerts are updated periodically to notify users of important changes to the system and often provide links to instructional PowerPoint’s demonstrating new functionality. 10

15 The Hospital Contact for each facility maintains the Hospital Profile Hospital Characteristics define each facility for the creation of peer groups when running reports. 15

21 Hospitals can select the default measures predetermined by their State or Network. Click here to select Network 21

23 Quality Health Indicators Entering Data

24 Data elements populate the data entry screen based on measures selected in the Hospital Profile. These elements are required to calculate those measures.

26 This indicates data is verified and ready for inclusion in reports. After completing and verifying all monthly data. The Activate Data for Reporting box is checked. Always click Save following data entry 26

28 Click icon to for more detail. Arrows provide a quick reference by indicating where the current entry falls relative to the target value. (Above, Below or On Target) Lowest EntryHighest Entry Target Targets can be set for each indicator to help hospitals monitor their progress.

30 Select Reports There are 5 types of reports available: Clinical/Quality Employees Financial/Operational Hospital Characteristics & Patient Satisfaction 30

31 Click to select option 1. Medication Omissions Report. Note: The list of reports is based on the data elements selected to collect in the Hospital Profile. In other words, if data for a particular measure is not collected by the hospital, a report for that measure cannot be run and is not presented as an option on the reports screen. 31

32 Note: Only the users facility name is shown. A randomly assigned number identifies other hospitals. Click the drop down arrow to view other report options. Reports default to this Basic Text Report view. 32 Hospitals can contact other hospitals directly from a report by clicking the envelope icon next to their randomly assigned number. Both parties receive an advising there is a new message in their message center.

33 To print this view, click the Just Print button. The report pops up in a new window for printing. The printed report is titled and date stamped. For easy access to reports a user frequently generates, a list of favorite reports can be saved to My Reports by clicking Add to My Reports. 33

34 Specific reports were designed for hospitals participating in the Multi-State Benchmarking Project. These reports are presently identified as Multi-State reports 34

36 The data can also be exported to Excel for display in charts, graphs or spreadsheets, etc., as needed. 36

37 The Message Center tool presents two options for communication. It allows users to communicate with the QHI System Administrator or with other hospitals directly from Reports To send a message to the System Administrator 1.Click on the Help/Suggestion menu. 2.Click on Enter or View Your Suggestions 37
