1 Quality Health Indicator Demonstration April 28, 2005

2 Access the QHI website from the Kansas Hospital Association Home Page

3 A user name and password is required to enter this secure web site. The level of access is determined by the user type.

4 The Welcome Screen provides contact information and dates for data entry deadlines. Click My Profile

5 Each user maintains their own user profile.  System Administrator  State Administrator  Network Administrator  Hospital Contact  Hospital User  View Only Level of access is determined by the User Type.

6 From Administration, select Hospital Profiles The Administration menu options reflect the level of access of the user. Hospital Contacts, for example have access to User Profiles, to add users at their facility and Hospital Profiles to maintain the contact, demographic and characteristic information for their facility.

7 Drop down lists assist in the completion of this required information. The Hospital Contact for each facility maintains the Hospital Profile Click Hospital Characteristics define each facility for the creation of peer groups when running reports.

8 Hospitals can select the default indicators predetermined by their Network. Click here to select Network Hospitals can also select additional indicators to meet their individual facilities needs. Indicators automatically populate. Select Data to begin entering data into the site.

9 Select Month or Add next Month Targets can be set for each indicator to help hospitals monitor their progress.

10 Red question marks ? provide pop-up definitions. Data is entered into boxes provided.

11 Click here to for more detail. Arrows provide a quick reference by indicating where the current entry falls relative to the set target value. (Above, Below or On Target)

12 Lowest EntryHighest Entry Current Entry Click to close Target

13 The Activate Data for Reporting indicates data is verified and ready for inclusion in reports.

14 Select Clinical/Quality There are 6 types of reports available: Clinical/Quality Employees Financial/Operational Patient Satisfaction & Hospital Characteristics System Clinical / Quality

15 Click to select option 1. Medication Omissions Report. Clinical / Quality

16 Note: Only the users facility name is shown. Reports default to this table view. Click the drop down arrow to view other report options. Clinical / Quality

17 All reports can printed for distribution.

18 Selected monthly facility data can also be exported to a comma delimited file and saved in Excel Format.

19 Profiles can be created for data that is exported routinely. Select a previously saved profile from the drop down list. Settings are automatically populated.

20 The exported data can be displayed in charts, graphs or spreadsheets, etc., as needed. Data can be imported to the site similarly.
