2 Accessibility The MED e-care login screen is accessed from a
local internet service through a secure hyperlink supplied to you at the time of installation. User needs to enter a valid user name, ID and password in the text fields provided. Click Sign-in button to log into the application

3 Getting Started Once the above process is successful, next screen
will prompt you to select the facility to access. From the drop down menu select the facility you want to logon to i.e ZenQA and ZenQA CCRS and then click Continue option.

4 Screen Terminology… The next screen is the system
“Dashboard”. This houses a calendar section for scheduling, a bulletin board, reminder, and is the launch pad for all of the Software systems available in e-CARE IV. Each facility has the ability to customize the Dashboard to include the facilities logo, hyperlinks and portals to valuable websites and resources, such as MED e-care Online Support.

5 …Screen Terminology To access the Message center, select the View
Message Center from the above figure. The Message Center allows users to communicate with the other users of e-CARE IV. It is similar to most of the standard systems i.e select the user you want to send a message, key the details of your message and send. You may retrieve and read messages; you have received from other users. It will also show you what users are “online” or currently logged in to the system.

6 …Screen Terminology Your system administrator
will have the ability to post announcements or bulletins on the Dashboard for all users. To post announcements, select the Announcements from the right side panel as shown.

7 …Screen Terminology The e-CARE IV application
contains multiple systems that user can access. Systems that are available to the user are dependent on how the user’s security has been set up and checks whether they have security rights to access or not. When user clicks on e-ADT and e-Assessments it allows you to access the Admission, Discharge & Transfer and Assessment systems. The e Reports system is where you run reports for all systems. System configuration is setup by clicking on Settings, and clicking on My Account will enable you to change your password.

8 e-ADT Overview The e-ADT (Admission, Discharge & Transfer) system is where you document a Client’s demographic information, contact information, client health and advance directives. It has the ability to track all admissions, discharges and transfers from your facility. Admission allows the users to admit a new client and check the details of the current admissions. It includes the following options: Create a new admission View or edit current admissions Perform a re-entry (re-admitting a client that was previously discharged) Transfer a Client (including internal transfers and bed swaps) Discharge a Client

9 New Admission… Setting up a new client is done through e-ADT system
in the e-Care IV application. This module assists users to admit a new client, discharge and transfer.

10 …New Admission From the left pane, click Admission/Discharge
tab and then, click New Admission. New Admission information is displayed on the screen. This will enable the user to create a new admission, view or edit current admissions, re-entry of a previously discharged client and discharge a client.

11 …New Admission Select the Health Sector from the drop down menu
provided i.e Complex continuing Care, General Rehab or Mental Health. You may either select an existing client from the Patient Selection Screen and then click on Admit Selected or if the client does not exist in the database, select Admit New Client For a new admission, the next step entails entering the client identification, contacts, client health status, responsible for payment and admission information. Certain fields in this record are mandatory and must be filled in Note: Fields marked with the symbol are mandatory

12 …New Admission In this screen, the user needs
to fill all the Client details. This information includes: Client Identification Contacts Client Health Status Advanced Directives Admission Information Client Identification The first tab is the Client Identification i.e the client’s biographical information. Chart numbers may be manually entered in, or you may have the system select the next available chart number. You also have the ability to upload a picture of the client. Once you have completed entering the information on this screen, select the Contact tab to continue.

13 …New Admission Client Contacts There are 2 areas on the
Client Contact screen. On the top pane you may either select from an existing list of funeral homes or create a new funeral home contact. If your client has made any arrangements with a funeral home prior to admission, you may enter that information here. The bottom pane allows you to enter the Client’s personal contact information, such as emergency contacts and next of kin.

14 …New Admission To create a new contact for your
client, select the New Contact button from the before figure. It navigates to the contact window as shown. Fill the appropriate information, including the type of relationship the contact has with the client as well as the associations and personal information and click on the save button to save the contact information. To close the window, select the close button.

15 …New Admission Client Health Status: The next tab of information
that needs to be filled in is the Client Health Status. This screen allows you to select Allergies your client may have, Treatments and Medications they are receiving as well as a Diagnoses. Check the boxes and enter the descriptions in the fields displayed.

16 …New Admission To add Diagnoses select the
Add button on the bottom left of the screen. This will launch a window that enables you to select from over 200 different diagnoses. Check the boxes from the left pane for selecting appropriate diagnosis. To jump to diagnoses that start with a specific letter, select one of the Alphabet buttons on the bottom pane. You also have the ability to search by the name of the diagnosis by using the Search option at the top of your screen.

17 …New Admission Admission Directives: The next tab is where you can
select the payment responsibilities for your Client. Fill additional information in the fields provided.

18 …New Admission Admission Information: The Admission Information
tab allows you to enter details about your client’s admission such as the program they are being admitting into, admitting and primary Physician, bed assignment, and referral source. Fill all the admission details of the client and click either Pre-Admit Admit. Check the Signature and click on the Admit button to complete the admission process.

19 Current Admission… To View or Edit a Current admission, select Current
Admissions from the left pane. It displays list of all admissions in the Patient Selection Screen. You may further filter the Patient Selection screen by Selecting a Filter, using the Search by feature or filter by a letter of the Alphabet on the bottom of the screen. Select the client name in order to view or edit the Client Information.

20 …Current Admission To Edit the Client’s Information,
select the Client Name and edit. Select Update button to update the client information details.

21 Transfers There are 3 types of transfers available. Transfer
Internal Transfer Bed Swap The Client Transfer information updates MAR administration which removes the Client and Medication from the administration screen Transfer From the left pane select Transfer and then select the Client name you need to transfer. Notes can also be included regarding the transfer. Check the Signature area and select the Transfer button at the top of the screen Note: Signature is Mandatory

22 Internal Transfer An Internal Transfer of a
Client to a different unit in the facility can be accomplished by Selecting the Internal Transfer option. From the left pane select Internal Transfer option and select the client Name you need to transfer Select the New Nursing Unit and New Bed Notes can also be included regarding the internal transfer Check the Signature section as Complete and click on the Transfer button

23 Bed Swaps From the left pane select Bed Swaps option. Select the
first Client Name from the search box provided. Chart number and Bed number are automatically generated by the system. Repeat the process for the Second Client and click Swap button in order to save the details.

24 Discharge To discharge a Client select the Discharge option from the
left pane and then select the client name you need to discharge. Fill the details with appropriate information. Check the Signature section and click the Discharge button to complete the discharge.

25 Re-Entry Re-Entry option is used to re-enter the Client’s
information which was stored in the previous database. Select the Re-Entry option from the left pane and then select the Client name which was previously transferred Fill the details with appropriate information Check the Signature section and click the Re-Entry button to complete the re-entry of the client The system will re-admit the client with the original profile information

26 History Client history information is available by selecting the
History option from the left pane. The following are the options available: Admission History Re-Entry History Discharge History Transfer History Internal Transfer History Select the type of history you would like to view or edit and then select the client name. Click Edit button in order to edit the history details of the client. Once the client has been admitted through the e-ADT system, they are automatically scheduled for the appropriate pending events. In most cases, the first pending event after a new admission is an Admission Assessment.

27 e-ADT Reports… Select e-Reports to Access the reporting system.
On the left pane it displays the different types of reports. Select the appropriate report and click ok to generate.

28 …e-ADT Reports Once the report selection has been made it displays
parameter screen. Each report has a parameter selection. Select the applicable parameters and click on the Run Report button. Report description can also be displayed by clicking on the Report description button. By default, reports run in PDF format.
