What is Wellness? Wellness is the state of good health achieved by balancing your physical, mental, emotional, and social health.

How to Assess Wellness One of the best ways to assess your overall wellness is to take a health assessment A health assessment is a set of questions that allow you to evaluate each of the four parts of your health.

5. Health Assessment Take the following health assessment. Score each question using the following Almost never Sometimes Almost always

Health Assessment Do you always eat a well-balanced diet and avoid foods high in fat, salt, and sugar? Do you participate in vigorous exercise at least three times a week? Is your weight within the normal range for your age, height, and sex? Do you feel good about yourself? Do you set aside time each day to relax

Do you avoid drugs, alcohol, and tobacco? Do you get about eight hours of sleep each night? Do you have close relative and friends you can talk to? Can you express your feeling in healthful ways? Do you use a seat belt every time you get in a car?

Do you use sunscreen when participating in outdoor activities? Do you make your decisions based on your own values and goals, rather than those of others? Do you avoid high risk behaviors? Do you practice good hygiene? Do you wear a helmet when riding a bike, skateboarding, or snowboarding?

Health Assessment Score Total up your all your health assessment points. Score your answers using the following point system: Almost Always = 3 Sometimes = 2 Almost Never = 1

Health Assessment Use the chart to find your current level of wellness. 38-45 points: You have great health habits 29-37 points: Your health habits are good overall, but there is room for improvement 22-28 points: Many of your health habits need work 15 -21 points: You are taking unnecessary risk with your health

5. Health Assessment Review Pick one statement from the health assessment that you scored an “almost never”. Explain what you can do to change this behavior. How will it improve your health?
