 Proofread the transcript mailed home in the summer  Complete applications (almost always online)  Send applications and fees  Send SAT/ACT/AP Scores ◦ Your SAT/ACT scores will not be reported on the Phillipsburg High School Transcript  Make an appointment with your counselor  Request teacher recommendations  Complete Senior Questionnaire

 Signed and addressed Transcript Release Form  Addressed envelope ◦ Colleges admission’s department address and Phillipsburg High School’s address as the return address ◦ No stamp necessary  Secondary School Report Form (Counselor Recommendation Form)- Not always required ◦ Be sure to fill out your portion ◦ Sign/date (if applicable)  You must hand in all materials DIRECTLY to your counselor! Do not leave in mailbox

 Available in the office & online  In order to complete the form you will need to provide the following information:  Name and address of the college the transcript is being sent to  Deadline for the transcript to be sent  Signature of a parent/guardian if the student is under 18 The Guidance Office will need 10 business days in order to process the transcript and send it out. Be aware of school vacations and holidays and school deadlines.

 Counselors will mail the following items:  Transcript  Secondary School Report Form  Counselor Recommendation Letter  Midyear Reports (grades) will automatically be mailed to each college! ◦ Students do not need to give us additional forms
