SAFE WORK INSTRUCTIONS Title of Subject: Strata Proximity PAD SAFE WORK INSTRUCTIONS The following safe work instructions is NOT INTENDED to replace hands on training requirements of the code of federal regulations. The intent is to provide an overview of known potential safety hazards associated with equipment operation. Hands on training with an experienced operator on each specific model must be conducted. The trainee can not proceed onto production until they are determined ready to proceed by the supervisor of the training. Before any operation or maintenance procedure is performed with or on this equipment, it is important that personnel have a complete understanding of the location and functions of all controls and indicators, as well as, the location and use of all emergency shutdown controls and procedures. Steps for Initial SWI Training: Prepare – Review the SWI. Present – Quick Card at the machine with experienced operator. Tryout – Person runs the machine with supervision. Follow up – Competency points are verified by supervisor and 5000-23 is signed by the trainee/trainer to complete the training. SAFE WORK INSTRUCTIONS Title: Strata Proximity PAD SAFE WORK INSTRUCTIONS KEY POINTS SAFETY PRECAUTIONS Pre-ops: Outside Checks by Security Ask Light house personnel if the PAD has been checked out using the COS Check in/Out station. PAD must respond to each zone (WARNING and HAZARD) with an audible and visual alarm. All button combinations must be tested. Battery level will be checked. Proximity Pads must pass all checks or taken out of service. Pre-ops: Outside Checks by Mine Personnel Perform visual Inspection of PAD on the surface before taking underground: Check for cracks in the PAD Buttons missing on PAD Missing light cover. Any Visible damage to PAD Pre-ops: Underground Checks by Mine Personnel Once on section visually inspect all proximity hardware: Generators, Controller, POD, Cables, Junction Box, Silent Zone Loop (if required), etc. Test Proximity PAD with the machine power on (pump Not energized) Proximity generator will show blue LED Proximity POD or TREE should show Blue LEDs and RED LEDs at the initial startup Perform a communications check by depressing S1 on PAD. System should respond with 2 flashes of Yellow Warning Zone LED on POD and on each Field Generator Make sure system hardware and cables are intact and not damaged. A successful communication check will flash the yellow warning light twice.

SAFE WORK INSTRUCTIONS Title: Strata Proximity PAD SAFE WORK INSTRUCTIONS KEY POINTS SAFETY PRECAUTIONS Pre-ops: Underground Checks by Mine Personnel Validate Magnetic Fields’ with machine pump motor activated the Blue LED on the display POD and Green LEDs at each Field Generator should be illuminated if each Generator is healthy. The Blue LED on each Generator will also be illuminated Make sure system hardware and cables are intact and not damaged. Check Warning and Hazard Zones: Warning zone for each generation is set at 4 feet Hazard Zone for each generator is set at 2 feet Once on section visually inspect all proximity hardware: Generators, Controller, POD, Cables, Junction Box, Silent Zone Loop (if required), etc. Test Proximity PAD with the machine power on (pump Not energized) Proximity generator will show blue LED Proximity POD or TREE should show Blue LEDs and RED LEDs at the initial startup Perform a communications check by depressing S1 on PAD. System should respond with 2 flashes of Yellow Warning Zone LED on POD and on each Field Generator

SAFE WORK INSTRUCTIONS Title: Strata Proximity PAD SAFE WORK INSTRUCTIONS KEY POINTS SAFETY PRECAUTIONS Mount/Operation/Tramming: Maintain 3 points of contact Sound audible alarm to alert others of your intended movement. Check service brake for proper function Ensure that tram roads are damp to wet to comply with the dust control plan. Never let passengers ride Ensure all persons are in the clear prior to tramming. Utilize all operator protection. Travel at a safe rate of speed for the conditions. Set outriggers on pads or on a level, solid surface and set brakes. Use wheel chocks on sloped surfaces when it is safe to do so. Do not excess the load-capacity limits Do not use aerial lift as a crane. Do not tram with the lift platform raised. Do not exceed vertical and horizontal reach limits. Do not operated an aerial lift in high winds above those recommended by the manufacturer. Note: Be aware of overhead clearance and overhead objects, including ceilings. Elevating Platform Check work area and observe proper safety precautions while working on the raised platform. Be sure all persons are in the clear. Be sure the area above and below the platform are free from obstructions Do not allow cables or hoses to become entangled in the lift. Remain within the confines of the railings and never over reach. Personnel on the ground are not permitted to operate the platform while it is occupied except in an emergency. Never use the structure of the platform as a welding ground.   Note: Disconnect both battery leads before performing any welding operations

SAFE WORK INSTRUCTIONS Title: Strata Proximity PAD SAFE WORK INSTRUCTIONS KEY POINTS SAFETY PRECAUTIONS Parking & Shutting Down: Ensure equipment is parked in a safe location. Park in non operating area or designated parking place. Retract and lower the attachments until contacting ground. Ensure that all levers are off and centered. Put Battery models on Charge Utilize three points of contact when entering or exiting the equipment. Do not block walkways or park too close to objects or other equipment.
