Title of Subject: Man Bus/Personnel Carrier Last Reviewed Date: October 2015 SAFE WORK INSTRUCTIONS The following safe work instructions is NOT INTENDED to replace hands on training requirements of the code of federal regulations. The intent is to provide an overview of known potential safety hazards associated with equipment operation. Hands on training with an experienced operator on each specific model must be conducted. The trainee can not proceed onto production until they are determined ready to proceed by the supervisor of the training. Before any operation or maintenance procedure is performed with or on this equipment, it is important that personnel have a complete understanding of the location and functions of all controls and indicators, as well as, the location and use of all emergency shutdown controls and procedures. Steps for Initial SWI Training: Prepare – Review the SWI. Present – Quick Card at the machine with experienced operator. Tryout – Person runs the machine with supervision. Follow up – Competency points are verified by supervisor and is signed by the trainee/trainer to complete the training. Title: Man Bus/Personnel Carrier SAFE WORK INSTRUCTIONS KEY POINTS SAFETY PRECAUTIONS Work Area Examination: Examine the roof and ribs for loose materials and the walkways for potential slipping/tripping hazards. Scale loose roof and ribs as necessary. Remove potential slipping/tripping hazards. Preoperational Checks: PA State Law - Diesel operators must have received initial 8 hours of instruction by a diesel certified instructor. Approved diesel pre-operational checklist must be completed in it’s entirety prior to being operated. The number of passengers is limited by the number of SCSR’s and seating located on the man bus. The man bus must be maintained free from accumulations of combustible materials.(coal, dust, paper, grease and oil). Any materials transported in or on a mantrip must be secured and stored in a permanently designed location. Sound the audible alarm prior to starting the man bus to perform any of the pre-operational checks to alert others in the area of your intentions. Ensure that the man bus is in a safe location prior to beginning the checks. Do not cross between man buses to perform the checks. Be alert to traffic in the area. If any of the items on the checklist cannot be completed the items must be repaired prior to operation. Maintain three points of contact at all times when entering or exiting the man bus. Prior to starting the man bus to perform any of the checks ensure that all personnel are in the clear. Any materials transported inside of the man bus cannot be transported in the same side of the man bus as passengers. Tramming: Sound the audible alarm prior to starting, tramming or when approaching turns or switches to alert others of your intended movement. Lights must be on in the direction of travel. Ensure that all personnel are in the clear prior to starting or tramming. Slow down when approaching turns or switches. Ensure proper switch alignment prior to travelling thru switches.

Title: Man Bus/Personnel Carrier SAFE WORK INSTRUCTIONS KEY POINTS SAFETY PRECAUTIONS Tramming Continued: Safety chains must be connected in the passenger compartments prior to tramming. Maintain a minimum 300 foot travel distance between rides. Follow the site specific block light policy, mine safeguards and haulage travel patterns. Man bus must be operated at a speed consistent with the conditions. In the event of a man bus derailment the responsible person must be notified prior to re-railing. Sound the audible alarm whenever approaching miners in a crosscut, shelter hole, curves and intersections. Slow down and sound the audible alarm when flashing blue lights are near the haulage. The operator of the man bus is responsible for the safety of all passengers in the man bus. Ensure that all switches are returned to there original position after travelling thru. Operators and passengers must keep all body parts inside of the man bus at all times. When it becomes necessary to wait in a switch for loads or empties to pass by, all individuals are required to dismount the man bus and position themselves in a designated safe location unless the man bus is parked in a designated safe location or oncoming traffic is travelling with the switch. (Policy) Flashing blue lights indicate that work is taking place near the haulage. Do not pass by miners that are not located in a crosscut or shelter hole. Parking: Whether temporarily or permanently parking the man bus the engine must be shut off. Ensure that the man bus is parked in a safe location and the parking brake is set prior to exiting the man bus. Diesel equipment cannot be left unattended with the engine running. Maintain three points of contact when entering or exiting the man bus. The man bus must come to a complete stop prior to entering or exiting.
