Title of Subject: Fall Protection-Inspection/Donning/Usage SAFE WORK INSTRUCTIONS Steps for Initial SWI Training: Prepare – Review the SWI. Present – Quick Card with experienced operator. Tryout – Person performs task with supervision in non production. Follow up – Competency points are verified by supervisor and is signed by the trainee/trainer to complete the training. Steps for Retraining: Review SWI or Quick Card Verify Competency Points Complete Annual Training Requalification Form Competency Points: Inspection © Donning © Usage © The following safe work instructions in NOT INTENDED to replace hands on training requirements of the code of federal regulations. The intent is to provide an overview of known potential safety hazards associated with equipment operation. Hands on training with an experienced operator on each specific model must be conducted. The trainee can not proceed onto production until they are determined ready to proceed by the supervisor of the training. Before any operation or maintenance procedure is performed with or on this equipment, it is important that personnel have a complete understanding of the location and functions of all controls and indicators, as well as, the location and use of all emergency shutdown controls and procedures

Title: Fall Protection-Inspection/Donning/Usage SAFE WORK INSTRUCTIONS KEY POINTS SAFETY PRECAUTIONS Inspection of Harnesses and lanyards Examine for damage (frayed, faded or burnt webbing, broken stitches, broken/cracked metal,) Examine lanyard housing and locking device Store fall protection in dry environment and out of direct sunlight Replace damaged components before use Never use fall protection equipment that been deployed in a fall unless equipment has been recertified. Donning Ensure properly sized harness Never altar fall protect system Choose the appropriate size for proper fit Don’t alter equipment (Punch holes, change buckle, etc) Usage Establish recovery plan prior to deploying fall protection equipment Examination of work area (Anchorage point, fall area) Never utilize break away lanyard (always use self-retractable lanyard) Always make sure you are tied off 100% and anytime working above 6 feet Anchor point must be rated to 5000lbs; Ensure anchor point is above work area Never work alone if fall protection is utilized Always utilize fall protection when working above 6 feet. Note: Ensure person receives fall protection instruction during part 48 training by a qualified instructor.
