(continued)  Addendum/addenda will be issued with clarifications/changes  Proposals will be evaluated per the Submittal Requirements as stated in the RFP  All proposals are due PRIOR TO AND NO LATER THAN :  3:00 PM, January 15, 2016  Contract Term – July 1, 2016 – June 30, 2017 with four (4) one-year renewal options and optional 1-6 months if needed at the end of the option year.

Representations and Certifications  The completed R&C page, becomes page two of the proposal  Non-Profit Status Proof & subcontract requirements  Disabled Veterans Business Enterprise requirements  Certification of Debarment, Suspension, Current Cost or Pricing and Independent Pricing  Name, Title, Date & Signature

Contract Conflict Certification  The completed Contract Conflict Certification Form, becomes page three of the proposal  Attach disclosures to certification form or declare “None”  Company/Organization  Name, Title, Date & Signature

Indemnification Certification Form  The completed Indemnification Certification Form needs to be submitted with the proposal
