4 The proponent must provide written certification from the State of Georgia that he/she is a certified/licensed asbestos contractor in the State of Georgia. Each Proponent shall have a minimum of (3) years of documented experience in asbestos abatement. The Proponent must demonstrate ability to perform asbestos abatement activities by submitting evidence of the successful completion of training courses covering asbestos abatement. The Proponent shall furnish proof that employees have had instruction on the dangers of asbestos exposure, on respirator use, decontamination and OSHA regulations. Proponents must supply a list of names of the employees who have completed the referenced training courses with evidence of successful completion. The Proponent must possess written standard operating procedures and employee plans which include specific reference to OSHA medical monitoring and respirator training programs. In addition, the Proponent must be prepared to make available for viewing at the job site, a copy of OSHA regulations at 20 CFR 1926.58 governing asbestos controls and Environmental Protection Agency regulations at 40 CFR Part 61, Subpart M, (NESHAPS) governing asbestos stripping work practices and disposal of asbestos waste. 4

Volume I – Information drafted by Proponent Executive Summary; Organizational Structure; Resumes of Key Personnel; Overall Experience; Management Plan; and Exhibit A-2; Specification Requirements. B.Volume II – Required Submittal Forms (Forms 1-9) II.Cost Proposal: Exhibit A.1 (Proponent must submit (1) stamped “original” and eight (8) copies in a separate envelope. Note for Submission of Responsive Proposal Packages: (1) Proposal Package Should be Marked “Original” and eight (8) additional Copies of the Original Proposal package must be submitted. 10
