Student Accountability Demographic Information: Answer Document Use no. 2 pencil only, erase changes completely. Check for extraneous marks on answer documents. All documents must be checked in an effort to reduce demographic verification at a later date. The test administrators name must be written on answer document EXACTLY as it appears on Group Information Sheet (GIS). The same name must be entered on online SGL.

English Language Learners The testing requirements for ELL have changed: ELL First Year Excluded – There is no blanket exclusion of ELL students. Exclusions must be individually determined based on guidelines listed below: ELL students may have their scores excluded from the calculations for AYP Accountability for all Achievement & Gateway/EOC assessments if they: a) are identified on the Home Language Survey as having a home language other than English, and a) are identified on the Home Language Survey as having a home language other than English, and b) score limited English proficient on the Tennessee ESL Assessment, and b) score limited English proficient on the Tennessee ESL Assessment, and c) are in their first year of enrollment (less than 365 days) in a U.S. school c) are in their first year of enrollment (less than 365 days) in a U.S. school NOTE: ELL Excluded Bubble should only be darkened if the student meets ALL of the above guidelines. Immediately Report a potential breach of test security to the System Testing coordinator and the State Assessment, Evaluation, and Research Senior Executive Director. The system and School must conduct an immediate and thorough local investigation of the incident. If NO breach is found: Notify the State Assessment, Evaluation, and Research Senior Executive Director Re-train staff on Test Security Law and local policies; consider revision of policies and procedures to avoid further problems. If a breach IS found: Contact and review results of investigation with the State Assessment Evaluation, and Research Senior Executive Director. Once a breach is confirmed notify the State Assessment, Evaluation, and Research Senior Executive Director. Immediately suspend the offending personnel and report this action to the Director of Teacher Licensure at (615) Student Test Impacted. Make Test Processing recommendations (Process Normally, Nullify or Void) with an online RI and a Breach form. Student Test Not Impacted Complete a Breach of Testing Security Report form with documentation of the investigation and results, including actions taken, and submit the form to the State. Review local policies and procedures for revisions and re-train staff to avoid further infractions.

Student Data Verification - When After system has completed scanning Student document review prior to scan provides less data verification concerns & will reduce this time. Before system receives quick scores Will receive notification once data has been loaded to Schoolhouse Data Verification Website

Student Data Verification - How Detailed Instructions Before beginning, Read Record Changes section in User Guide for complete description of buttons & functions used when making student record changes. Note: When you hover cursor over a button, a brief description of button's function is displayed. 1.Select Student Data sub-tab. The Student Data screen displays list of your students. 2.Select student record you wish to edit by clicking on student name link. 3.The Edit Student page displays that student's record and fields that can be changed. 4.After making changes, click on Save Changes button. To go back to student list, click on Return to Student Data Page button. 5.After all changes have been made, or if you had no changes, click on Finish Edits sub-tab. Then click on Finish Edits button. Note: Stoplight in upper left corner is now green indicating successful completion of process. 6.You can go back & update student edits any time before deadline. 7.Select Student Roster sub-tab to view or print original or current student roster. 8.Select school you wish to view or print by clicking on Customer Selection drop-down menu & highlighting your school. 9.Select either Original Student Roster or Current Student Roster link. Note: The report will be in MS Excel format.
