Steps for Applying for Initial Licensure: STEPS 1-4 Step 1: Please go to to explore information from the National Association of Boards of Pharmacy (NABP) that will be invaluable to you through this process! Step 2: Download and read the NAPLEX/MPJE Registration Bulletin available at mpje-bulletin pdf. mpje-bulletin pdf Step 3: Set up an e-Profile with NABP (available from nabp.net at Step 4: Set up an account with Pearson VUE at: Once you have an e-Profile, you can choose to register for NAPLEX and MPJE. You should select SC 121 Presbyterian College School of Pharmacy from the School Code List when submitting your NAPLEX and MPJE registration. The current fees associated with registration for these exams are as follows: – NAPLEX $505 – MPJE $210

Tips for Completing Your State’s Application: What is Your Name? It is very important that, on all forms related to applying for you pharmacist license, you list your name EXACTLY as it appears on the government issued identification (ex: your driver’s license) that you intend to present when you arrive for testing.

Tips for Completing Your State’s Application: College Affidavit and Certification of Clinical Experience The Dean will be completing: – the College Affidavit (found on the top of page 6 of the South Carolina application) with College seal. – Certification of Clinical Experience (found on page 8 of the South Carolina application). The above will be provided to you by the Dean at the time of your graduation. – These forms cannot be completed until you have successfully finished your last rotation and your grades have been entered and verified. The student is responsible for completing all other parts of the application.

Helpful Facts for Completing the South Carolina Application…

Practical Experience (Page 4) List practical experience you have had under the supervision of a licensed pharmacist in chronological order. These are your INTERNSHIP HOURS, NOT YOUR IPPEs or APPEs.

College Clerkship or Rotations (Page 4) There is insufficient room to list more than 4 of your rotations on the form. So, you will need to do one of the following: 1) List your first four rotations and then write (see attachment). On a separate sheet of paper, list the remaining rotations in chronological order. – Be sure to put your name on the separate attachment. 2)Or, you could write “see attached” and then print your rotation list from E*Value and attach that. This print- out will include your name and all of your rotations in chronological order.

Previous Examination Record Previous Licensure as Pharmacist (Page 4; Practical Experience) If none, state N/A Full disclosure: If you have ever taken the Board examination for pharmacist licensure in any state, you must disclose the place(s), date(s), and your result(s).

Record of Charges, Convictions and Fines Imposed on Applicant (Page 5) Please complete the statement as listed on the form. Be sure to sign this portion of the form. You must have the form notarized by a Notary Public.

Proof of Qualifications: Birth Certificates Obtain a certified copy of your birth certificate (this will not be returned to you). Character Vouchers (Page 6) You will likely want to wait to have this completed after you receive the completed College Affidavit portion which is the top part of this page. Character Vouchers A&B will need to be completed and signed by licensed pharmacists. The signatures for character vouchers A&B must also be the signatures on the (A) and (B) lines on the back of the pictures (Page 7). One picture is submitted with the completed application. The other picture must be presented by you when you arrive for testing.

Certification of Clinical Experience (Page 8) Will be completed by the School and signed by the Dean and provided to you upon graduation. If you are applying for licensure in another state, you will need to complete this form and send to: South Carolina Board of Pharmacy PO Box Columbia, SC Or hand deliver to: 110 Centerview Drive, Suite 201 Columbia, SC 29210

Affidavit of Eligibility (Page 9 and 10) Please check the acceptable secure and verifiable document(s). A copy of the verifiable document(s) must be attached to the Affidavit of Eligibility.

Application Submission Submit your COMPLETE Application for Examination by: Mail: South Carolina Board of Pharmacy PO Box Columbia, SC Or hand deliver to: 110 Centerview Drive, Suite 201 Columbia, SC Be sure to: – Pay the $100 fee. – Enclose a copy of your birth certificate. – Enclose one picture with signed attachment. KEEP the second picture and signed attachment. You will need these for admission for testing. – Enclose the Certification of Clinical Experience signed by the Dean. ---Your licensure application is valid for ONE year.---

After Your Application Has Been Approved By Your State’s Board of Pharmacy…

Scheduling Your Exams: Pearson VUE Pearson VUE will provide your Authorization to Test (ATT). The ATT provides scheduling information you will use to schedule your NAPLEX and MPJE. You will not be provided an ATT to allow scheduling of your exams until your application has been approved by the state you are seeking licensure with. Pearson VUE will also provide you with a confirmation letter which will list the details of your scheduled exam time, location, etc.

Once you have successfully passed the NAPLEX and MPJE…

Initial Licensure Process Once you’ve passed both the NAPLEX and MPJE, you will receive a Pharmacist’s Initial Licensure Application and information sheet. Complete and submit both forms as well as a $70 initial licensure fee. You must complete this process NO MORE THAN 30 DAYS after receipt of the forms.

Becoming Licensed in More Than One State… NAPLEX Score Transfer Program

Score Transfer Please see: transferhttp:// transfer When you register for the NAPLEX you select the primary state or jurisdiction where you are seeking licensure If you take advantage of the NAPLEX Score Transfer program, you may have your NAPLEX score transmitted to an additional state(s). – Requests may be submitted up to 90 days after sitting for the NAPLEX. – You can select additional jurisdictions during the exam registration process or submit score transfer requests separately after you sit for the scheduled exam. Cost: $75 per transfer Must take MPJE (if required) and meet all other licensure requirements for each state in which you wish to become licensed. See each state’s BOP for details (
