Purpose To inform the TSM (Transitioning Service Member) of the health care and mental health care options available to them and their family.
Standard Before the member separates they should get: a physical a dental checkup copy theirs and their family members’ health records
Separation History and Physical Examination (SHPE) Comprehensive medical evaluation Required for ALL separating service members Documents existing medical conditions 180 days prior to separation
DoD Mental Health Self-Analysis Program Mental health/alcohol screening and referral program Voluntary and anonymous Online, phone, special live events
TRICARE Offers full range of services for mental health and substance abuse treatment First eight weeks of treatment do NOT require preauthorization if within the network
inTransition CONFIDENTIAL coaching program Answers questions about treatment options Information about community resources Secures appointments at new location
Veterans Affairs Combat Veteran Authority offers enhanced enrollment for five years after discharge File a claim and document disability as soon as possible Disability does not necessarily need to be service connected for care
