3. Purpose: 40-To explain the importance and timeframe for the SHPE to identify any service connected medical conditions that require ongoing care 41-To ensure eligible TSMs are aware of the Transitional Assistance Management Program(TAMP)

4. Separation History and Physical Examination(SHPE)
Helps to establish service connection for medical conditions developed during active duty that require ongoing care
Separation Health Assessment(SHA), conducted through the VA using the Benefits Delivery at Discharge(BDD) program, meets SHPE requirement
Documented on DD

5. Separation History and Physical Examination(SHPE) cont.
Required for all active duty TSMs prior to separation
Also required for Reserve Component(RC) or National Guard that have served 180 days on active duty or 30 days in a contingency operation prior to release

6. Service Treatment Record(STR)
Medical and dental records make up the STR
Must be maintained by Military Treatment Facility(MTF) or medical department in order to meet requirement for future VA benefits

8. Transitional Assistance Management Program(TAMP)
Called “transitional” because it offers Tricare-like benefit for
180 days post separation
SM and eligible family may enroll in TRICARE Reserve Select
(TRS) or other medical insurance program
TSMs are eligible once they leave active duty

9. TAMP(cont.) and Eligibility Criteria
Transitional health care is NOT an automatic benefit
Disenrollment from TRICARE Prime is automatic upon separation
Sponsor and family are covered under TRICARE Standard unless
they re-enroll for TRICARE Prime during transitional period
Sponsor and family must be enrolled in Defense
Enrollment Eligibility Reporting System(DEERS)
AC and RC must meet criteria for eligibility

10. Health Insurance Marketplace
Helps uninsured Veterans and family find health coverage
If TSM is enrolled in TRICARE or VA Health Care Program,
they are considered covered under the health care law

11. Dental Coverage During TAMP
Provided on a space-available basis
Members will not receive dental care if an exam was
performed within 90 days prior to separation
VA provides one-time dental care for Veterans that apply
within 180 days of separation

12. Community Based Warrior Transition Units(CBWTUS)
Permits a recuperating SM to receive medical care at
facilities near home
Primary mission is to provide access to care for RC
Could result in evaluation board upon completion of
treatment

13. Medical Care for Retirees
TRICARE Prime requires an annual enrollment fee
TRICARE Extra requires a TRICARE network provider
and the Retiree to pay annual deductible
TRICARE Standard is a fee-for-service option and
requires the enrollee to file the claim
TRICARE for Life for family members that become
entitled to Medicare Part A

14. Survivors Family members are entitled to TRICARE benefits
as transitional survivors
Sponsor must have died while serving on AD for
a period greater than 30 days
Survivors will only pay cost shares and deductibles
at the AD family member rate
