2. DOES THIS APPLY TO ME ?
Must be completed by all active duty service members prior to Separation
Required for reserve & National Guard members that serve 180 days on active duty or 30 days in a contingency operation

3. WHY SHOULD I ?
Examinations are useful to support any ongoing needs for medical care
Establishes service connection for any medical conditions developed either prior to separation or in the future

4. HOW DO I WIN ?
Service members are to keep medical & dental records maintained with appropriate DOD authority
A properly maintained record is a requirement for future VA benefits
Failure to do so could affect future benefits

Transitional Assistance Management Program (TAMP) offers TRICARE- like benefits for a limited time
Available for service members and their eligible family members
Benefits are good for 180s during transition

8. AM I ELIGIBLE ?
Involuntarily separated with a separating program designator (SPD) that designates benefits
Member separated after being involuntarily retained for contingency operation
Member separated after voluntarily remaining on A/D for 1 year or less receiving sole survivorship & agreeing to enroll in SELRES, RR or a RC

9. ANYTHING ELSE ?
Community Based Warrior Transition Units (CBWTUS)-allows soldier to live at home to recuperate while still on active duty
Continued Health Care Benefits Program (CHCBP) –following the loss of coverage TSM and family members may apply for temporary, transition medical coverage
