Forms! Forms! Forms! Consent form – behaviour! Emergency Contact. Medical form (NHS number) – permission for administering calpol /antihistamine/nurofen or emergency treatment. Groups for bedrooms / groups for activities. Money.

Accommodation Stratford Youth Hostel Bedrooms - Two floors - some en-suite! Food Snacks/ drinks – no chewing gum

Sweat shirts These have been ordered and will be given to the children the day before our trip. We will name them all and leave then in their place with their yellow cap ready for collection on the Sunday morning. Following the trip the children are welcome to wear them for school.

Kit List – to be named! Socks / shoes –VERY, VERY, comfortable – not new/ 2 named water bottles, preferrably sports caps. Underwear Medicines – must be named, prescribed and dosage written clearly on front– girls medicines to be handed to Mrs. Symondson / boy’s medicines to Mrs Platt. Trousers Shorts T-shirts Jumpers / fleeces Insect repellent! Waterproof Sun-tan Cream – application! Pyjamas Rucksack – should have water bottles ,waterproof, camera, suntan cream and lunch in it. Smart 0utfit for theatre –long trousers Towels Camera – recommend disposable - not digital and also needs naming! Washbag / toiletries – roll on deodorant
