Biosolids Annual Report A Step by Step Guide to Submittal

1 2 1) Make sure your “Site” is selected and 2) Click on “Apps, Requests and Reports”

Click on “New and Draft Compliance Reports”

Search for “Bio”

1) Find the right Annual Report, 2) Check for proper Due Date, 3) Click “Begin Submission” 1 23

Click “Begin Form Entry”

Click “Next Step”

Fill out the top part of Section I (tonnages)

Continue all the way to the bottom (names and addresses), and Click “Next Step”

Fill out Section II, and Click “Next Step”

Fill out Section III and, Click “Next Step”

Click “Select Attachment” and upload the required document (supplied template or contractor’s load sheets)

1) Make sure your attachment saved, and 2) Click on “Next Step” 1 2

1) Note that you have to fill out an Annual Report DMR and 2) Click on “Select Attachment” to upload analytical data 1 2

1) Make selections, 2) Attach certification statements, and 3) Click “Next Step” 2 3 1

Certify, sign and certify, and click on “Next Step”

Review

Review, click “Next Step”

Click “Submit Form”

Click “Return to Portal”

1) Make sure your “Site” is still selected and 2) Click on “View DMRs” 1 2

1) Find the Biosolids Annual DMR (Hint: Date range is 1 year), 2) Click “Open” 1 2

Note that there is nothing under “Daily” tab, Click on “Summary” tab

This is a place holder, but it will ask for Biosolids Analytical Data, trust me.

If you land applied, 1) Enter Biosolids data, 2) Click “Save,” and 3) Click “Validate and Review” 3 2 1

If you didn’t land apply, 1) Check the “No Discharge” box, 2) Click “Save,” and 3) Click “Validate and Review” 3 2 1

Click “Certify and Submit” and you have completed your Biosolids Annual Report in MiWaters!
