2011 January to March - All Incidents

Derrick & Well Ops Incidents

Lifting diverter through rotary, overshot + extension (4.5 MT) detached and fell 50ft to jacket deck Cover for top drive junction box (5kg) detached during jarring & fell 70ft to rig floor – sheared bolts Lifting out annular packer (680 kg) from LMRP it struck an anode and fell 9m to deck Sledge hammer fell 40ft to rig floor; ring on end of tool lanyard failed Picking up wire-line valve (130kg) to run into well, it detached from running tool & fell 4.3m to rig floor

Crane & Lifting Operations

Wooden sheathing from crane boom rest dislodged & fell to off-load hose walkway Lifting valve from WI filter package, actuator detached and fell 30” to grating below (corrode bolts)

Other Dropped Objects Two lengths of redundant deluge pipe-work (7kg & 2kg) detached and fell 6m to walkway Emergency beacon (10kg) detached from bracket, falling 2.5m to extent of electric cable Hammer head (1.8kg) detached and fell 13m during rope access work on crane boom Valve being manoeuvred struck second valve which slipped under handrail & fell into sea
