The pipe had fallen due to broken corroded support brackets (3 out of 4). The pipe had to be re-positioned before the skid could be offloaded. No crane or forklift at site until next day. Team was in a rush to return the MT truck and decided to lift manually. Broken & Corroded Support

During lift with 5 man: the last support bracket broke. The very heavy pipe struck one of the crew. He badly broke his leg. He will be off work for several months .

What caused this LTI: 1) Poor inspection/maintenance of the pipe supports 2) Improper manual handling: should have waited for a crane or forklift. 3) Failure to analyse the Hazards of this non-routine job. 4) Time pressure to complete the task and return the mt truck for another load. Discuss in your team: - Do you ever lift heavy objects manually? Why? Is this not the task for a crane or forklift? - Is there time pressure that makes you cut corners on safety? - Do you discuss hazards (what can go wrong ?) before starting a new routine or non-routine job?
