6 Derrick & Well Ops Incidents Breaking flanged connection between HP riser & tension table,, stud and nut fell 25m to platform deck Running liner, mud-saver valve (32kg) on “OverDrive” connected with liner tool joint & fell 13.5m to rig floor Pulling pipe, top-drive protection frame struck stand of racked drill-pipe; piece of frame fell to rig floor Picking up drill-pipe single to stab into string, elevators opened; joint fell 1.5m to rig floor Rigging down wire-line equipment, IP suffered head injuries when sheave parted from lubricator & fell 10m

8 Crane & Lifting Operations Booming down, headache ball struck keeper bar which was dislodged & fell into sea Crane boom struck redundant comms equipment; dome (2.1kg) fell 30m to generator roof Spooling main crane rope; rope socket failed; length fell 35m to deck Sheave guide (26kg) fell 45m to deck from main block of crane Lowering flange (95kg) on blue polyprop rope, flange fell 3m to deck

10 Other Dropped Objects Section of cladding (8kg) dislodged from cabin module in high wind & fell to walkway Section of deluge pipe (30kg) detached & fell 15m to compressor roof; corrosion of pipe and fixing points Corroded metal plate detached from turret & fell 4m to deck Floodlight (27.6kg) fell 9m to module below after being struck by abseiler Windsock (1.5kg) dislodged by helicopter downdraft, falling 10m to palfinger gantry

11 Other Dropped Objects Scaffolders moving equipment; 2 scaffold boards (2 x 9kg) knocked 26m into sea During work on riser, spanner (3kg) fell 8m to scaffolding below Fitting blind to well flow-line; blind (3.5kg) dropped, falling 4.5m to cellar deck Scaffold board dislodged from scaffold, falling to deck below Slip plate (1.1kg) dislodged by work party & fell through gap in grating 8m to deck below
