2. Cause A crane operator and 2 riggers were sent to de-rig a kiln section. 1 rigger determined that they may need a set of pliers, so he left to go get them. The remaining rigger proceeded forward with the de-rigging. The shackles on the crane side were removed first, and then the rigger moved to the side opposite the crane to try and undo the rigging on that side. The employee was moving in and out of the operator’s sight, alternating flagging the crane to boom down and trying to ensure that the cradling chokers did not get hung up. As the rig boomed down, the craneside chokers crept up the side of the kiln section until they rested on top of it. The crane continued booming down, allowing the chokers to fall down the opposite side.

4. Training Opportunity Recordable 1-23-07 Improvements Employees must always consider whether or not they are placing themselves in the line of fire. Riggers will cut loose on the opposite side of operator first and complete the process of de-rigging on the operator side whenever possible (This allows for an extra set of eyes). The importance of wearing toolbelts at all times will be stressed. The crane hook needs to be directly over the lifting point when disconnecting in the future.
