Separation History and Physical Examination Ensure to get a copy of your medical health records for you and your family It’s a requirement 180 days prior to their separation Separation History and Physical Examination

The DOD Mental Health Self-Assessment Program Mental health, alcohol screening and a referral program Voluntary and anonymous Depression, bipolar disorder, alcohol use, PTSD and anxiety disorder Active duty members can self-refer For family members the first 8 visits do not required pre-authorization if it’s within the TRICARE network The DOD Mental Health Self-Assessment Program

Each facility offers clinical & non-clinical prevention, education, treatment programs and campaigns. Post-deployment Assessments conducted at 6 months, 1 year, and 2 year intervals.

DOD’s INTRANSITION Program Confidential Coaching Program If SVM was seen by a behavioral health provider within 1 year of separation they will be automatically enrolled in the program No expiration date to enroll DOD’s INTRANSITION Program

Veterans Affairs Coverage Combat Veteran Authority- enhanced enrollment program Reservist who served on active duty Treatments on PTSD and substance abuse Former service members with dishonorable or “bad papers” discharge Veterans Affairs Coverage
