The Five Axes of the DSM-IV Axis I: Clinical disorders Axis II: Personality disorders and mental retardation Axis III: General medical conditions Axis IV: Psychosocial and environmental problems Axis V: Global assessment of functioning (helps assess prognosis) The major clinical disorders and adjustment disorders are on Axis I. Axis II disorders - Characteristics of an individual’s personality or abilities. Personality disorders - Personality traits that are inflexible and maladaptive and that cause either subjective distress or considerable impairment in a person’s ability to carry out the tasks of daily living. Axis III - Clinician records the client’s medical conditions using the medical profession’s diagnostic numbering system, the ICD. Axis IV - Clinician documents events or pressures that may affect the diagnosis, treatment, or outcome of a client’s psychological disorder. Axis V - Used to document the clinician’s overall judgment of a client’s psychological, social, and occupational functioning. Global Assessment of Functioning (GAF) scale: Axis V of the DSM-IV, a scale that rates the individual’s overall level of psychological health. Community mental health centers (CMHCs): Outpatient clinics that provide psychological services on a sliding fee scale for individuals who live within a certain geographic area. Halfway houses: Community treatment facility designed for deinstitutionalized clients leaving a hospital who are not yet ready for independent living. Day treatment program: Structured program in a community treatment facility that provides activities similar to those provided in a psychiatric hospital. Family therapy: Psychological treatment in which the therapist works with several or all members of the family. Group therapy: Psychological treatment in which the therapist facilitates discussion among several clients who talk together about their problems. Treatment Implementation Course of Treatment The clinician’s role in treatment The client’s role in treatment Outcome of Treatment Clinician’s Role - Quality of the relationship between the client and clinician is a crucial determinant of whether therapy will succeed or not. Client’s Role - Psychotherapy is a joint enterprise in which the client plays an active role. Describe and identify the nature of his or her disorder, to describe personal reactions as the treatment progresses, and to initiate and follow through on changes. Outcome - Client remains in treatment until the treatment runs its course, shows improvement, and maintains this improved level of functioning.
