The OK-SCREEN Login Page The KARES Login Page The OK-SCREEN Login Page Providers will submit an account application. When a user account is created, a username is entered. When the user account is saved, a password is auto-generated and emailed to the user. The password is encrypted and cannot be viewed in the KARES user interface. The user uses the username and password to log into KARES. When the user logs into the system for the first time, the user will be prompted to change the password and set a security question. If the user forgets the password, the user can enter his or her username and security question answer, and the system will generate a new password and email it to the user. Passwords must have: At least 8 characters (but no more than 16) At least 1 upper case character At least 1 lower case character At least 1 number At least 1 letter At least 1 special character

From the Home Screen the user selects ‘Applications’ to start a new entry.
And selects ‘Add New’ and enters the SSN and Last Name OR Date of Birth Note that KARES does not rely on your web browser’s back button. Instead use the back button on the screen or select options from the menu bar.
Since the applicant was not found we will add a new applicant
Last Name and SSN were carried forward. Next, enter the individual’s information CARRIED FORWARD
Both Aliases and Prior Addresses can be entered.

Let’s say Tim worked in Virginia and South Carolina previously. Select ‘Add New’

With the personal and demographic information complete we’re ready to check the licensure and registry status.
‘Add License/Certification’ button. If the applicant has a license not discovered in the system you may record that information by selecting the ‘Add License/Certification’ button.
The red asterisks indicate required fields for this option.
Enter the License/Certification information and ‘Save’
Click the ‘Select’ box for a license/certification listed if it is held by the applicant, (2) required in order to be hired for the position for which the applicant is applying, and (3) has an ‘Active’ status. One or more items may be shown and can be clicked.

Next select the Provider, Position Category and Position from the context driven drop down lists.
Select ‘Next’ to move to the screening tasks.
The Research Registries screen expands based on the addresses selected The Research Registries screen expands based on the addresses selected. In this example, the nurse aide registries from South Carolina and Virginia have been added based on our earlier entries. Note that some registries are optional.
Where ‘Auto-Match’ is shown indicates that the SSN is automatically checked against the database. A zero indicates there were no findings matched on that SSN and the results may be marked ‘cleared.’

Here are the results from the OIG List of Excluded Individuals/Entities Search. In KARES, use the Ctrl-F4 key combination to close the browser window and record the results.
(although it is not a required registry). Registries that are not Auto-Matched are searched by clicking the hyperlink to open a new web browser window. Click on the registry listed (although it is not a required registry).
This search can be done based on Name and DOB.
There is not a match. In KARES, close the browser window and return to the KARES browser window. Hint: Press and hold the Ctrl key while pressing the F4 key to quickly close your current browser window.
‘Not Cleared’ or ‘Flagged for Review.’ If ‘Not Cleared’ or ‘Flagged for Review’ are selected, entries in the comment box are required and indicated by red asterisks. Click ‘Next’ to continue. Record the Research Results for all of the registries. The options are ‘Cleared,’ ‘Not Cleared’ or ‘Flagged for Review.’
A clean registry review generates the above Application Submitted Confirmation. Select the Final Registry Results form to print a confirmation report for the applicant record. A download dialog box for a PDF file format report appears.
Page 2 of this report begins the registry findings. This is page 1 of a 3 page report which includes identifiers for the provider, KARES, the applicant, and the applicant’s license(s). Page 2 of this report begins the registry findings.

You can also print a Live Scan Fingerprinting Form, a Fingerprint Authorization Form, and a Self-Disclosure Form. Note that the applicant’s information will be automatically carried over into the forms.

You may now track the application by clicking “Determination In-Process”
The last names are hyperlinked to view the record or the user can withdraw the record. This global view shows multiple records. You will only see the records applicable to your facility. Click on ‘Tester’.
