SCOPE OF WORK The improvements proposed under this LAP funded project includes the following, Mill 1” of the existing roadway on NE 2nd Avenue between George Bush Blvd. and NE 13th Street and resurface with 1” of SP-9.5. Widen the roadway to construct a 4 ft. green colored pavement bike lane on each side of the roadway. The widening cross section will be composed of a 12” stabilized subgrade (LBR 40- compacted to 98% max. density), 8” limerock base prime coated and compacted to 98% max. density, and 1.5” of SP-9.5 in two equal lifts. Swale regrading to improve drainage
SCOPE OF WORK The construction of a 5 ft. wide sidewalk on each side of the roadway and ADA ramps. The construction of decorative brick pavers pedestrian crossings. Signalization improvements at the intersection of NE 2nd Avenue and George Bush Blvd. that includes the installation of vehicle loop detectors and pedestrian detectors. Landscape and irrigation. Signing & pavement markings improvements that includes the relocation of various signs with the ROW.

MOT PLAN SPECIFIC TO THE PROJECT MOT to be set up as per plans sheet C-17 to C-24 and the FDOT standard Index 600. Phase I: reduce the SB lane width to 10 ft. and begin work on the west side of the roadway. Phase II: Reduce the NB lane width to 10 ft. and continue work on the east side of the roadway. Phase III: Use detours and lane closures as shown in the plans and index 600, 603 and 660 to M&R and the brick pavers crossings on George Bush Blvd. and on the side streets. Challenge: Drop Off Protection. Solution: Inspect the work at the end of day to ensure drop offs are protected. Travel Lane treatment with 1:4 slope. Challenge: Maintain pedestrian access during construction Work only on one side of the roadway Use sidewalk closed signs at legal and safe designated pedestrian crossings. Protect pedestrian drop offs during construction Challenge: Notification of lane closures in advance Solution: Install two Dynamic Message Signs 14 days in advance of each MOT phase.
MOT PLAN SPECIFIC TO THE PROJECT Challenge: Driveway Access during construction Solution: Notify the driveway owner at least 14 days in advance Work on one driveway at a time per property. PROJECT EXPERIENCE – LAP Projects Florida Department of Environmental Protection – Kemp Channel Bridge Contact: James Post, Project Manager Telephone: 305.853.3571 Email: james.post@dep.state.fl.us Scope of Work: Project FM #41839-1; (LAP Funded); Construction of the Kemp Channel Bridge Span revamps the Overseas Railroad Bridge over the Kemp Channel into a pedestrian walkway. It is located at approx. MM 23.5. It includes replacement and retrofitting of three missing spans and railings, comprising 116 feet of the long abandoned bridge between Cudjoe and Summerland Keys. The span will be about 15-16 feet above the mean-high waterline, with navigational lights marking the channel for boaters. Other improvements include picket rail installation, milling and resurfacing, join cleaning and resealing, rail retrofit and pavement stripping. PROJECT EXPERIENCE – LAP Reporting to FDOT Track and record the contractors’ quantities as per the FDOT requirements. This includes collecting and filing material deliver tickets (i.e. asphalt and concrete), and any required testing reports (i.e. earthwork densities and concrete plastic properties). Reviewing contractor’s material submittal to ensure compliance with the FDOT LAP Requirements (Buy America, concrete/asphalt from FDOT approved sources) Accurate record keeping is a necessity for LAP Projects; these records are scrutinized during the FDOT LAP audits. LAPIT Uploads. Monthly Reimbursements reviewing contractor’s invoices to ensure that they include the required forms (Certification of Previous Disbursements & Certification of Compliance with Plans). At the end of the project, the LAP Close-Out Documentation (Blue Folder) will be assembled and submitted by the CGA Staff.
