23 Cumulative SAA Issues (FY Q1 ’12 – Q3 ‘15)

24 Significant Safety Achievements- 5/15 A significant amount of work was completed setting up the new work activities in the Activity Manager system. –A total of 99 work activities have been generated for ETA. –There are zero work activities remaining in the collaboration (draft) stage. All are either “final” or pending in “EHS collaboration”. –64% of ETA personnel have been assigned to one or more work activities and have accepted. Another 17% have been assigned but are pending worker acceptance. –Approximately 500 ETA personnel have been removed from the old JHA system. –Four Activity Hazard Documents (AHD) have been canceled and are now covered by Activity Manager. The quarterly SAA inspections were completed on 5/8/15. Several issues were identified and corrected. The annual update of the ETA Integrated Safety Management (ISM) Plan has been completed.

25 Short Term Safety Outlook 6/15 Complete all ETA work activities in Activity Manager. The remaining 33 activities in “EHS Collaboration” need to be finalized. Remaining assigned workers also need to be finalized. ETA Safety Committee will be held June 5. Complete the Glove Box Operation Self-Assessment project report for submittal. Complete an old chemical/sample clean-out in the 70-264 lab area. Continue to address ETA equipment on the electrical equipment “fail” list Assist with the set-up for new lab areas 70-143, B33 JCESR, and B60 test chambers. Participate in the upcoming City of Berkeley hazardous waste inspection. Participate in EHS Division’s Reactive Chemical Use self- assessment.
