F AMILY C ONNECTIONS Resources: About Me/Careers/Colleges Resume Building (9 th ) Personality Profile (10 th ) – connects careers and possible majors in college College & Scholarship Search (11 th – 12 th ) Customized data relating to SHS students. Supports college planning and the application process Linked to colleges worldwide and their websites. http://connection.naviance.com/sherwood Username: Email address. Password: Student ID# (6 digit ID#).

5 C OLLEGE APPLICATION PROCESS 1. Select the colleges for the application process. 2. Update your list of colleges on Family Connections under the tab “ Colleges I’m Thinking About.” 3. Know your deadline dates for each college (early action/early decision, rolling, priority, regular, etc.) 4. Fill out applications – prioritizing by deadline dates. 5. Submit applications to the colleges. 6. On Family Connections, move the colleges that you have applied to under the tab “Colleges I’m Applying To.” This is where you will enter your deadline date, choose your application type and request transcripts, mid-years and letters of recommendation online. 7. After you submit an application, 15 school days are required to process your transcripts (paper copy to the registrar) and letters of recommendation. Remember to speak to your teachers requesting recommendations.

6 C OLLEGE APPLICATION PROCESS CONTINUED 8. Be sure to complete the FERPA waiver on the Common Application website if you are using the Common Application. 9. Enter your email address for the Common Application under the tab “Colleges I’m Applying To.” 10. Be sure when you turn in the paper copy of the transcript request form to the registrar you also bring any secondary/counselor reports and fees if needed. 11. The paper copy of the transcript request form is required because it is our back up system for what was entered online. 12. Request test scores to be sent to colleges from SAT (www.collegeboard.org.)/ACT (www.actstudent.org)

Listed below are the procedures to initiate the process. Submit request for transcript and letters of recommendation on Family Connections Complete and submit a College Transcript Request Form. Be sure to fill in the following: Student Name (use the student’s full legal name) School Counselor Name Student Birth Date and ID Number Parent Signature (Required if student is under the age of 18) Print the complete name and address for each school requiring a transcript. Indicate the application type for each request. List the application deadline date that corresponds with the application type. All documentation must be submitted to the Registrar’s office at least three weeks (15 school days) prior to the deadline dates. The first two transcript requests are free. All additional transcripts are $5.00 per request. Payment may be made by cash or by check payable to Sherwood High School. You can track your Transcript Requests on Family Connections.
