PRACTICAL ISSUES Confirmation of Study (1) Name of Signatory: Karolína Kaslová / Daniela Slámová/ Vladimíra Žáčková Position: Exchange Student Coordinator Date: –Arrival Sheet: –Departure Sheet: all exchange students will be given a chance to finish their exams by the end of classes, i.e. May 13, 2016

PRACTICAL ISSUES Confirmation of Study (2) Name of Host/Receiving Institution: University of Economics, Prague Erasmus Code: CZ PRAHA09 Period of study: to later ONLY in case of registered exams during the exam period: to last day of a student’s duties = last exam

PRACTICAL ISSUES Learning Agreement = LA (1) Agreement between 3 parties –Student, home uni and host uni Student agrees to study the courses Home uni agrees to accredit the courses in the full degree programme back home Host uni (VŠE) confirms that the courses are in line with its course catalogue

LEARNING AGREEMENT = LA (2) Who is responsible Your university issues ALL documents How to complete your LA: 1.Responsible person at home institution has to agree with the courses 2.Fill in your personal data, courses and sign it 3.Signature of institutional/ departmental coordinator 4.Signature of host university coordinators (Karolína Kaslová/Daniela Slámová/Vladimíra Žáčková)

CHANGES TO THE LA (3) All changes have to be approved by the responsible person at home university FIRST Write all final changes to „Section to be filled during the mobility“ (aka Changes) Forward us the approval of changes by your home uni OR bring it to RB547 to have it signed

PRACTICAL ISSUES Learning Agreement (4) Needs to be: –Signed by the home uni first –Brought or sent by to Exchange Office (RB 547, check the Visiting Hours on website) –Amended when a new course is added (Section „Changes“), preferably after the end of the registrations
