4. Exam Check Out Form If you plan to leave early, you need to fill out this form. Please make sure your name and number are printed neatly. Check the class periods in which you are leaving campus after the exam. Need Parent Signature For example, if you have a first period exam on Tuesday, but not a 3 rd period exam, you would check 10:15 (1 st ) – that is the time you are checking out.

5. How can I leave early? Fill out the exam check out form. Turn in the form to the attendance office or to your 1 st period teacher. Make sure your parent or guardian has signed the form. Once the form is turned in, you do not need to go to the attendance office to check out during exam week. The form is all that is needed. If you do not have a 1 st, 2 nd, or 5 th period exam, then you do not have to come to school until 10:00am on your exam day if you can get to school at that time. IF YOU RIDE THE BUS HOME, YOU MUST REMAIN ON CAMPUS UNTIL 3:30! School buses will run on a normal schedule. They will not arrive to school late or leave campus early. If there are any days when you have no exams, then you don’t have to come to school that day!

6. More Info… You may not leave an exam period early. Once the exam period starts, all students are required to stay until the end of the exam period. Students that have not completed the Bridge Bill requirements will not be able to check out until the work is completed.

9. Scenarios It is Tuesday. Your 7 th period teacher told you that you don’t have to take an exam. When do you need to be at school? When can you leave campus? What can you do from 1:45 – 3:30? What happens if you ride the bus? What would happen if you did have to take the 7 th period exam?
