CCG Feedback January 2014 Issue/QueryOutcome/Update District Nurses have ceased to visit house bound patients to take blood tests This has been raised with the CCG’s Contracting team who are in discussions with SERCO. It has been identified that there is a commissioning gap which the CCG are seeking to fill ASAP. QOF QP 2013/14The QOF QP pathways are: Outpatient Referrals; ALL, OA Hip and Clinical Thresholds. Emergency Admissions; ACE3 tool, Admission Prevention Services and Diabetes A&E attendances; Practices will need to develop and implement an improvement plan. All reports need to be submitted to: Queries can also be sent to this address. Please note, whilst the new Integrated Diabetes Service is not yet live, there are a number of actions that practices can take to ensure that they have engaged with the pathway. The briefing for this is included in the delegate NHS 111 UpdateFollowing national feedback, the number of duplicate messages to GPs from 111 have reduced, which will cut down on paperwork in surgeries. The secondary post-event messages, sent when a patient is referred to a GP OOH service to reduce message duplication, has now been supressed. A workshop has been arranged to understand the causes better and to explore options for further improvements. A similar situation exists with the Wellbeing Service line which will also be explored. Ipswich Hospital has extended services over the weekends with increased presence of senior clinical decision makers.
