7 NHS West Kent Clinical Commissioning Group In 2015 -16 our plans are to develop.. New Primary care New Secondary Care Mental Health Services Integration with Local Authorities

8 NHS West Kent Clinical Commissioning Group We are developing New Primary Care by: Coaching clinical Microsystems to support primary care Integrating GP Out of Hours Services and Hospital at home Services Introducing the Care Plan Management System to help clinicians share information about vulnerable patients Supporting GPs to develop Federations or similar networks of practices Piloting schemes to develop teams of community nurses and social care staff integrated with groups of practices Reviewing diabetes services with a view to implementing a Level 3 Specialist Community based Service, integrated with Federations of GPs

9 NHS West Kent Clinical Commissioning Group We are developing New secondary Care by: Transforming outpatient services and the use of Kinesis Developing a long term service strategy with MTW to ensure a sustainable future for them and for the CCG Developing an integrated Urgent Care Service that brings together A&E and non-elective inpatient activity in new ways.

10 NHS West Kent Clinical Commissioning Group We are developing Mental Health Services by: Improving crisis care in line with the Concordat Increasing the diagnosis and support for Dementia sufferers Continuing to develop IAPT services Developing improved services for children and young people with MH problems by developing and implementing with NHS England and KCC; an Emotional Wellbeing strategy, a Neurodevelopmental Pathway (including ASD and ADHD), and Eating Disorders.

11 NHS West Kent Clinical Commissioning Group We are integrating with Local Authorities by: Continuing to develop joint commissioning arrangements for LD, Children’s services, and Emotional Wellbeing Developing an understanding of the total budgets spent by all agencies in West Kent - a total place approach to commissioning. Working with District and Boroughs to develop a primary care strategy that supports spatial development and regeneration activity
