Why we need to change Our population will grow from 1.6 million people to 2.2 million by 2031 People are living longer New treatments, drugs and technology are expensive

More will be carried out locally e.g. in GP surgeries Some specialist centres We will share our skills and what we know We will work together to improve services for the older people and those with long term conditions such as diabetes, heart disease or breathing difficulties Our vision
