2 Aims of Today We want to have an open and honest debate about health care in Stoke-on-Trent We want for you, our public, to understand and inform our direction of travel and have a meaningful say on our priorities for the future We want to work together on the bold actions needed if we are to build an excellent local NHS, now and for the future We want local people back in charge and for you to know that we are listening

3 Just to be clear… Preserving the values that underpin a universal health service, free at the point of use, will mean fundamental changes to how we deliver and use health care services. This is NOT about: Privatising the NHS Charging for services Restricting access This IS about: Meeting the demand for healthcare Making ever penny count Ensuring people have the right care, at the right time, in the right place.

4 Nationally… In 1948 the NHS was founded, and it now treats 1,000,000 people every 36 hours. As a nation: We are living longer, and have an increase in our ageing population We are living with long-term conditions; and the number of people living with more than one long-term condition is on the rise The price of care is rising within a challenging financial environment nationally We are expecting more from our healthcare providers

5 In Stoke-on-Trent… We are the 16 th most deprived local authority out of 326 in England We have a lower than average life expectancy than other cities in England The three main killers are: circulatory disease respiratory disease cancer A third of adults live with one or more Long Term Condition (LTC) We use more beds than other cities with the same level of elderly and ill patients, which costs us a lot more money

6 In Stoke-on-Trent we spend… In 2013/14 the CCG’s planned expenditure is £350 million And demand for services will continue to grow and grow, but our funding will not…

7 To put our spend into context… What things cost on average… GP Cost per Appointment: £25 A&E attendance: £105 Short Stay UHNS admission - 0 days: £675 Long Stay UHNS admission 1+ days: £2,100 Community Hospital admission: £7,000 Intermediate Care (step up or down): £1,400

8 Our Priorities: We want to be… Helping people to stay well Helping people to avoid health crises Supporting people to live independently at home Supporting people when they need it most Working better together Getting the most out of the money we spend on healthcare

9 To do this.. We’re rebuilding our services around what people tell us they want: independence, care in their own homes or nearby, hospital care only when they need it We’re tearing down professional barriers so you get all the health & social care support you need in one place We will help you get and stay healthier so you won’t need care in the first place, and help you to be as independent as you can be All this is to create a better service at a more affordable cost

10 We are already doing things better Integrated Local Working Bringing together GPs, nursing, social care and community and voluntary services Early help so that people can manage their health and prevent deterioration Intermediate Care Joined-up services supporting patients to be safely looked after at home To help patients to be discharged from hospital when they are ready to go home with short term nursing and if needed social care

11 We are already doing things better Frail and Complex Support Improved services and people equipped with the knowledge and skills to support and meet the needs of older people Geriatrician support and care into the community, supporting community teams When in hospital people receive specialist care that gets them the right treatment for their needs, in the right place at the right time – first time Other areas Services being better organised for patients from urgent and emergency care through to daily care in the community One plan for workforce, with one plan for finance

12 But we still need you… Our Challenges/Your Voice Thinking about our priorities, how strongly do you agree or disagree with our ambitions? How should we, the CCG, be doing things differently? Tell us what makes the difference in great, quality care locally? Hospital Care and Care Close to Home Using your own experiences, how can we reduce the demand on hospital services? What would make you feel confident when receiving care closer to home? Self Care/Prevention What can we do to support you to look after yourself and your health?
