Derrick & Well Ops Incidents Breaking flanged connection between HP riser & tension table,, stud and nut fell 25m to platform deck Running liner, mud-saver valve (32kg) on “OverDrive” connected with liner tool joint & fell 13.5m to rig floor Pulling pipe, top-drive protection frame struck stand of racked drill-pipe; piece of frame fell to rig floor Picking up drill-pipe single to stab into string, elevators opened; joint fell 1.5m to rig floor Rigging down wire-line equipment, IP suffered head injuries when sheave parted from lubricator & fell 10m
