9 Profile Form details: SIMON Medical Module New Role Created; Maintainer – Medical System The first section is the header of the profile. This requires Medicare information. Medical Contacts – Section is required, Types are customizable. Medical Items are grouped in their respective categories. All items need to be reviewed. Choosing yes to an item may present additional options e.g. file uploads (based on it’s configuration). PAGE 7
