Please see the following screen shots for additional assistance Log in to Ascentis For password reset or troubleshooting please call Ascentis @ (800) 229-2713, ext. 8, option 1 Select “My Team” from the icon menu in the upper left hand of the page. Select “My Direct Reports” from the left hand drop down menu Your employee names will appear, followed by their Job Title, Hire date, employment status and email Select an employee by hovering over the name and clicking the mouse. A small window will pop up with your employee’s picture and information. Click on the “Attachments” tab on the left hand side of the small pop up window. At the top of the new window, you will see a “Show” drop down menu. Drop the menu and choose “Benefits – Immunization Attachments” . All scanned immunizations will be listed by date and test under the “Description” column. Choose the immunization, click on the file name and print up the .PDF for your records. Immunizations from 2016 forward will be scanned into the Ascentis file. If you do not see an immunization you are looking for (prior to 2016), please contact Occupational Health at x6949 You do not need a “release of information” form to access your employee files.
