Welcome & Congratulations! You are currently being considered and reviewed for membership in accordance with the rules and regulations of the NHS and the local chapter’s bylaws. Remember– you are in NO way guaranteed membership in the National Honor Society. Your application must be completed, in full, and handed in on time.

Consideration for Membership Candidate Acknowledgement Form This page acknowledges that you do indeed wish to be considered for membership. As indicated on this page, you and your parent/guardian must sign this form. This will ensure that both the applicant & the applicants parent/guardian have read and understood all of the directions and procedures involved in the selection process. ------------------------------------------------------------------------------------------------ Membership Contract This page outlines the requirements and expectation of a National Honor Society member. As indicated on this page, you must sign this contract.

Consideration for Membership Candidate Data Form This page indicates any academic, attendance, and/or discipline matters that have occurred throughout your high school career. You must fill out this form honestly and accurately. Checking “I have” to any academic, attendance, or discipline matters DOES NOT automatically disqualify you from consideration of membership.

Consideration for Membership Activities Data Form This page highlights your evidence of leadership and service, involvement in sports, and any honors received. Leadership – include leadership positions you have held inside or outside of school throughout your high school career. Service – list evidence of service (i.e. volunteer work) in school or in the community. You may include service completed starting from the summer before your freshman year to present. EACH ACTIVITY LISTED REQUIRES OFFICIAL DOCUMENTATION AS PROOF OF COMPLETED SERVICE ALONG WITH CONTACT INFORMATION OF YOUR ADVISOR. Honors – list any honors or awards you have received inside or outside of school.

Consideration for Membership Essay Form You will write one essay that includes both topics described on this form. The essay must be typed and no more than 500 words. Your essay is not part of your scoring, however it will be used to gain a better understanding of who you are as an applicant.

Consideration for Membership Recommendations Your recommendations are focused on the pillars of Leadership and Character. You will be required to have at least seven total recommendations – five teachers and two outside sources. You MUST include the names of your recommenders on the applicant checklist form. All building staff are automatically sent an e-mail with a survey monkey to complete. Your outside sources will fill out a hard copy of the recommendation included in your application packet. Outside recommendations should be MAILED directly to Ms. Filiaci. If necessary, these forms can be scanned and e-mailed to Ms. Filiaci at gfiliaci@frhsd.com. These recommendations are confidential and should NOT be viewed by the applicant.

Consideration for Membership FAQ’s How many hours of community service are required to apply? There are NO required number of hours to apply. The more the merrier. What qualifies as valid community service? Included in your application packet is a running list of activities that DO and DO NOT qualify as valid service hours. If there is an activity that you have participated in that’s not on the list and are unsure if it qualifies as valid hours, ask Ms. Filiaci (in room B203 or by e-mail: gfiliaci@frhsd.com). What qualifies as official documentation of completion of service? A signed letter from the supervisor of the service project, preferably on official letterhead, indicating a description of the service and the specific number of hours completed.

Consideration for Membership FAQ’s What if I don’t/can’t provide official documentation of service completion? If you do not provide official documentation of a completed service project it WILL NOT COUNT toward your application, NO EXCEPTIONS. Is my coach / club advisor / etc. considered a teacher recommendation since he/she works at the school? No. Your teacher recommendations must come from teachers who have actually taught you in the classroom (from any year). Your coach, advisor, etc. is used as an outside source and should complete the online form if they also work at the school.

Consideration for Membership FAQ’s What if my recommender forgets to fill out or send in my recommendation? As the application deadline approaches, be sure follow up with all of your recommenders with a gentle reminder that the recommendations are due no later than December 1st. If a teacher still neglects to fill out an application, and you have listed their name on your application checklist form, the NHS selection committee will follow up with the staff member during the time of selection. It is important to provide contact information of any outside sources so that the selection committee can follow up with these recommenders if they are missing.
