Character Scholarship Leadership Service NHS Information For 2017 – 2018 School Year March 8, 2017 Ms. Corbin, Mrs. Henderson & MS. JOHNSON NHS Advisers

Members are to be models of the four pillars of NHS: Character Scholarship Leadership Service

4 Candidacy for Membership
To be considered for membership in NHS: Submit an application to Mrs. Henderson (HP2) Application MUST be submitted to her room. DO NOT PUT IT IN MY MAILBOX OR SUBMIT TO OFFICE STAFF. NO LATER THAN FRIDAY, March 24, 2017 The Faculty Council will process the forms and students will be notified as to whether or not they have been selected for candidacy by Thursday, May 4th.

5 Criteria for Selection
Minimum cumulative EGUSD weighted GPA of 3.5 Personal Statement Letter of Recommendation From someone other than a family member or COHS teacher Can be elementary or middle school teacher All faculty will be asked to assess your character Rigor of Curriculum Copy of Transcript with AP/honors classes highlighted

6 Candidacy Period Once selected, an applicant becomes a candidate
The candidacy period runs from the time the applicant is selected for candidacy until the time the candidate is inducted as a member, during Term 2. Freshmen candidates may choose not to be inducted and reapply the following year. Once inducted, members who resign or whose membership is revoked may not apply again. In May, following the Induction Ceremony, inducted members earn their membership pin and card but must pay a $15 materials fee if they’d like them.

7 Responsibilities of Candidacy/Membership
Participate in the chapter’s yearly service projects and individual projects throughout the year. Earn a minimum number of 10 honor points per term (NHS group service, individual service, tutoring, scheduled functions, scout, church, mosque, and temple service, etc.). Attend all meetings. Meetings are two times per month at 8:30 a.m, on late start Weds in the gym A written excuse must be submitted for any absence. Students participating in sports, drama, or band need to have a note signed by their coach/director at the beginning of the season outlining the time frame they will be missing meetings.

8 Responsibilities of Candidacy/Membership
Remain in good discipline standing. Academic dishonesty, illegal activity, suspensions or expulsions can lead to permanent dismissal from NHS. Maintain a cumulative 3.5 GPA and no Ds or Fs GPA verified each term <3.5 GPA or a D or F or U in citizenship = warning notice and one term probation Students will be taken off probation when they meet the minimum requirements. 2nd consecutive <3.5 or D or F or U in citizenship will result in dismissal from NHS

9 Responsibilities of Candidacy/Membership
Members are expected to lead a Service Project (one large or two small) by the end of Term 3 of their senior year as well as to have been a member for 2 years to earn a sash for graduation recognition. Seniors who earn the sash must pay a materials fee of $20 if they’d like to receive it at Senior Awards Night. Failure to meet membership responsibilities will result in probation and/or dismissal from NHS. Students who are dismissed from NHS, may appeal the decision by submitting a letter outlining their reasons for appealing. This letter should be given to the adviser who will then meet with the principal.

10 Communication It is the responsibility of the National Honor Society members to keep apprised of any NHS news concerning meetings, events, service hours, honor points, etc. Information is posted regularly in School Loop and on our Facebook page, so it is imperative that students check these regularly. Additionally, our Communications Officer sends reminders with the app “Remind 101”. REMINDER: Phones are not to be used during class time.

Please complete all sections. TYPE all information and submit it by Friday, March 24, 2017. Turn in completed form to Mrs. Henderson (HP2). Do not be modest. Every bit of information you provide will be used by the Faculty Council to assist with the fair consideration of your candidacy during the selection process. Completion and submission of this form does not guarantee selection. Should you have questions about this form, please do not hesitate to contact Mrs. Henderson, Ms. Johnson, or Mrs. Corbin via School Loop. Did we mention…?...Application must be typed!

14 4. Personal Statement   On a separate piece of paper, please type no more than one side of one page, 12 pt. font, Calibri font, 1.5 spaced. You MUST use bullet points. Describe: Why you are interested in becoming an NHS member What you can contribute to NHS, Examples of how you have shown that you can represent NHS based upon the Four Pillars (the purpose and expectations of NHS Members): Character, Scholarship, Leadership, Service Be specific. You may go back two years.

15 5. Letter of Recommendation
Include a letter of recommendation from an adult who is NOT a family member or COHS teacher. This could be a coach, someone that you volunteered for, a pastor / imam/rabbi, scout master, elementary or middle school teacher, etc. It’s alright if your coach is also a teacher at COHS. Let the person know what NHS is about and what the Four Pillars (Scholarship, Leadership, Service and Character) represent. Ask them to return the recommendation letter to you and you will attach it to your application.
