Durham Connects and EDCI Durham Connects is a universal nurse home-visiting program with targeted efforts in the EDCI Zone. 174 EDCI Zone families have received a nurse home visit since % EDCI Zone families with newborns received a nurse visit last year 87% families referred to community resource or agency 65% families referred to Healthy Families Pilot one year follow-up visits with EDCI families will begin this year
