Consultation Forms 1 PAULA RAFFERTY

 A consultation is as important as the treatment its self, an accurate record must be kept along with details of any subsequent physical examination.  This questioning technique is known as the subjective assessment, which enables the therapist to build a picture of the client and what they aim to achieve from the massage.  Before questioning you should make your client aware of any H&S issues that are specific to your facility such as fire exits, fire procedures etc…

 Research a variety of consultation forms that are used by relevant companies (print the best ones off)  Using the forms as a model, produce your own that can be used when taking clients of your own for a massage session.  Remember to include a treatment plan and aftercare advise.  Once completed hand in for assessment.

 A contraindication is a specific situation in which a drug, procedure, or surgery should not be used because it may be harmful to the patient.  Massage treatment is non-invasive, relaxing and natural. It is therefore generally considered a safe treatment for most people. However, there are three types of contraindication:  TOTAL, when massage should not be performed at all.  LOCAL, when massage can be performed but not over the contraindicated areas.  MEDICAL, when massage can only be performed once medical permission has been granted.

Each group to research and complete a short presentation on one of the contraindications;  Cancer  Varicose Veins  Pregnancy  Skin Conditions  High/Low Blood Pressure  Sunburn.  You need to state which ones are Total, Local and Medical. You have one hour!

 With a partner fill in their details on your consultation form.  Once each of you have had a go at completing a client consultation form discuss!  Have you missed anything out?  Is everything under the relevant headings and following a logical format?  Amend as required ready for next lesson.

 10 minutes on each presentation.  Students ask relevant questions  Notes to be made on each one presented.  Next week everyone to hand in assessment for P4 which is:  Describe six contraindications to massage treatment.  Next week we will complete treatment plans which will conclude consultation forms.
