Objectives: Define the personal health record or PHR and the benefits it provides Describe 2 ways to create a PHR Identify what clients can do with a PHR Recognize client’s personal health information rights

3 PHR A PHR is an electronic record of health and medical information, which can be maintained by the client. PHRs contain a range of information including: Allergies and adverse drug reactions Family medical history Laboratory test results Hospitalizations and surgeries Medications and dosing Prescription records Vaccination records

4 How are PHRs created? Method 1:
Finding a tool that is downloadable, web-based or an App on a Smart Device. These tools can be free or low cost. In addition, information will have to be entered manually. Method 2: Using a tool offered by a healthcare provider or employer. These tools are free and should come pre-loaded with some of the client’s medical information. Clients can add and missing information and ensure existing information is accurate.

Benefits of PHR Benefits of using a PHR include: A client’s medical information is all stored in one place making it easily accessible It can be incorporated with a client’s healthcare provider’s records, helping physicians make better treatment decisions

6 What can be done with a PHR?
What can a client do with their PHR? Accuracy of Info: Check to make sure all information is accurate and complete. In the Know: Keep track of and print important health information such as vaccinations and test results. Availability: Have medical history available in case of a change of doctors or visit to a specialist. Medicine: Keep track of all medications and dosages.

7 HIPAA Client Privacy Rights Client’s privacy rights include:
Access - clients have the right to access their personal health information. Accuracy - clients have the right to correct or amend health information. HIPAA Disclosure - clients have the right to know who their personal health information has been shared with. Notice - clients have the right to be notified of specific privacy practices of healthcare providers and facilities. Complaint - clients have the right to file a complaint of Health Insurance Portability Act (HIPAA) violations.
