3 Online Surveys Purpose? To collect information from respondents Tool: Online survey tools –Free Online survey sites such as Survey Monkey Restricted features –Web 2.0 tools such as Google Apps Fully featured without the Glitz Reporting not as comprehensive

4 Survey Elements Relevancy 1.Questions are relevant to what you want to find out? 2.Are your respondents in a position to answer the questions? 3.Are you familiar with the questions? 4.Are you familiar with the objectives? 5.What information is needed?

9 Plan your survey Storyboard your survey on paper first, showing questions and question types etc..... This is easier than creating a survey on the fly....

10 Sign up to Gmail www.gmail.comwww.gmail.com sign up for a new account here if needed. Or logon if you are an existing Gmail user

11 Click on Documents to access Forms

12 Click on Create new : then select Form

13 Click here to give your form (survey) a title

14 First give your survey a title and information about your survey.

15 My first question is an example of a text based question. Respondents fill in their answer in a text box. I have provided some Help Text to assist in answering the question. I have also made this a required question. When satisfied click Done.

17 For your next question Click add item

18 Question 2 Example of multiple choice Click Add item and then Multiple Choice

19 Fill in your questions and choices Note I am making each question a required question. That means the respondents cannot move on unless the question is answered. Then click Done.

22 Fill in the checkbox question, help text and check box options Click Done when finished.

25 Write your question and choices Click Done when finished.

26 When your respondent clicks on arrow they can make a choice

28 Complete the question title, help text and choose scale number

29 Fill in your Columns and Rows for your question

34 Note: you can edit, duplicate or delete a question here.

35 Now my survey is complete: how do I distribute to my respondents?

36 You have three options: 1.Email a link to respondents 2.Place a link in a moodle web page 3.Embed a link in a moodle web page or label

37 Click on Email this form then enter email addresses Then click send.

38 To embed: click more actions and Embed

39 Copy the embed code: Control V

40 In a Moodle Block: click add a resource: insert a label

41 Click on Toggle HTML source button

42 Paste in the embed code and click the toggle button again

43 Click on Save and Return to course

44 Survey is now embeded in Moodle for completion

45 Respondents click submit when finished

46 When survey has been filled in click see responses

47 Summary is displayed
