3  Log-in to your Google Account  Click on the Google Apps Badge  Click on ‘Drive’ where all of your Google cloud storage exists

4  In your Drive, click on “Create” and then “Form”

5  When you open your new form, a screen will appear to prompt you to enter a new title for your form and choose a background  When done, click ‘OK’

6  First, enter the question in the box labeled “Question Title”  “Help Text” does not need to be filled out, but is used to supplement the question or give a hint when desired

7 -Text (Short Response) -Paragraph Text (Long Response) -Multiple Choice (only one answer can be chosen) -Checkboxes (multiple choices can be selected) -Choose from a list (Create a list of options to select from) -Scale (Rating/Enter highest and lowest titles for scale) -Grid (Enter a Table) -Date (Enter the correct date) -Time (Enter the correct time)  Next, click on ‘Question Type’ and select the type of question you wish to use:

8  Check out the ‘Advanced Settings’ for each question type! (Not mandatory)  Finally, select the box if you wish the question to be required (if an answer is not required leave the box unchecked)  Click ‘Done’

9  Option 1: Click ‘Add item button’ or Choose an option from the drop down list to start an entirely new question  Option 2: At the top right corner of the current question, select ‘Duplicate’ to enter the same type of question again

10 To reorder the questions, simply click, hold, and drag on the question you wish to relocate. You can drag the questions to any order on the form that makes sense.

11 Change the confirmation statement Choose to allow modfications  Click on the text box, and change the thank you statement as desired.  Select the check boxes to change whether you want respondents to see each other’s responses, or be able to make modifications after they have submitted their responses

12  When the form is complete, click on ‘Send Form’  Here, you can embed the form or share using facebook, twitter, email, etc.

13  To stop accepting responses, simply click on the ‘Accepting Responses’ link at the top of the form.  This will give anyone who tries to access the form an error message like the one shown here.

14  To view responses, simply click on the box titled ‘View responses’ at the top of the page.  Your responses will appear in a Google spreadsheet, stamped by date and time (not name!)

15  To view the live form, click on ‘View live form’ at the top of the sheet.  Live View will look like this one:
