16. OKLAHOMA STATE IMMUNIZATION INFORMATION SYSTEM I IMMUNIZE. SAVE LIVES. If you are unsure of the spelling of the patient’s name, type how it sounds, along with their DOB and use “Search with Soundex”

If multiple matches are found, results will appear as a list You will have access to three icons under the “Actions” heading which allow you to: View the Patient Summary View Patient Immunization Schedule Edit Patient Demographics

If only one result is returned, you will be taken directly to the individual’s Patient Summary screen Or their shot record From here you can print the individual’s schedule You can edit their Demographic Immunization You can view their immunization schedule

Icons in the left hand column reflect the status of the immunization series Current/ Up to Date Not IndicatedPast Due This is reflected in the “Status” column This is the last recorded date in OSIIS the immunization was given Recommended Date is the ideal date to administer this vaccine This is the earliest date the vaccine should be given This is the date at which the immunization would be considered Past Due

If you are navigating from a list of Patient Search Results, select the Edit Patient Demographics icon for the patient’s record you wish to own for your site Owning a Patient Record To own a patient record, you will first need to edit that individual’s record to reflect that they are active within your site

26. OKLAHOMA STATE IMMUNIZATION INFORMATION SYSTEM I IMMUNIZE. SAVE LIVES. Once you have updated the Site Status for all desired individuals, select Reports from the OSIIS Toolbar Running the Student Immunization Report

27. OKLAHOMA STATE IMMUNIZATION INFORMATION SYSTEM I IMMUNIZE. SAVE LIVES. On the OSIIS Reports screen select Student Immunization Report from the list of Available Reports
