2 ___1. Verify that all distributed secure materials have been returned. Notify the district coordinator immediately if any secure materials are missing and complete the necessary investigation. ___ 2. Make copies of and file the records of required administration information collected by each test administrator. (Page 21 of the manual) ___ 3. SECURITY CHECKLIST(S) with the records of security numbers from each test administrator. ___ 4. Make copies of and file the SSS FCAT Security Log(s). ___ 5. Generate a list of students who did not participate because they were exempt from testing. ___6. Generate a list of students who are enrolled, but who were absent the entire time. As You Receive WRITING+ TEST MATERIALS from the test administrators, do the following:

(continued) All NOT TO BE SCORED documents which includes: test books unused non-preidentified and preidentified answer documents answer documents with both DNS bubbles gridded ___ 8. If a document is NOT TO BE SCORED, verify that BOTH DNS bubbles have been gridded. If a DNS bubble has been gridded by mistake, verify that the DNS bubble has been erased and the corresponding UNDO bubble has been gridded, and place the document in the appropriate stack of TO BE SCORED materials.

6 Preparing Your School’s TO BE SCORED Answer Documents For Return ___9. Complete a separate SSS DOCUMENT COUNT FORM for each grade level tested. Verify that you have included all documents from make-up sessions before you complete the forms. DO NOT combine documents for more than one document type/grade level under the same DOCUMENT COUNT FORM. The Count Form is scannable and must not be photocopied. Place the completed Count Form on the appropriate stack of TO BE SCORED answer documents.

7 Complete the SSS ANSWER DOCUMENT RETURN SUMMARY __10. Complete the SSS Answer Document RETURN SUMMARY as instructed below. Every completed SSS DOCUMENT COUNT FORM must have an entry on the SSS ANSWER DOCUMENT RETURN SUMMARY. Transfer the information from each SSS DOCUMENT COUNT FORM to the SSS ANSWER DOCUMENT RETURN SUMMARY, making sure that the Number of TO BE SCORED Documents Returned for each grade level and document type is correct. Complete only one SSS ANSWER DOCUMENT RETURN SUMMARY. Verify that the number of documents returned and the number of documents indicated on the SSS DOCUMENT COUNT FORMS match the entries on the SSS ANSWER DOCUMENT RETURN SUMMARY.

8 Complete the SSS ANSWER DOCUMENT RETURN SUMMARY ___11. Complete the preidentified SSS ANSWER DOCUMENT RETURN SUMMARY FORMS which have been provided for each school. The following information has been preidentified on all Summary Forms. District Name and Number School Name and Number Grade Level and Content Area

9 Complete SSS Paper Bands __12. Each grade level and document type must be paper banded separately. You may use more than one paper band per grade level, if necessary. Follow these steps for each paper band you use: Grid the appropriate document type. Write the school name and district/school number in the appropriate place on the paper band. Count the number TO BE SCORED documents that will be banded in each paper band. Write that number on the line titled Number of Documents Contained in This Paper Band. Place a paper band around the appropriate stack of TO BE SCORED answer documents. Make sure the SSS Document Count form is on top of the banded stack labeled 1 of n. Be sure that the band securely holds the documents. Do not use staples, paper clips, or tape.

10 Preparing Home Education/McKay Scholarship Programs TO BE SCORED Documents For Return Process these documents separately from your school’s regular TO BE SCORED documents. Scores for these students will be returned to the District Office. Results will not be aggregated with district or school data.

11 Completing the SSS Document Count Forms for Home Education/McKay and Opportunity Scholarship Programs ___13. Complete the SSS DOCUMENT COUNT FORM by following the directions on the form. A separate Count Form must be completed for each grade level and for each program. Do not combine grade levels or Programs under the same SSS DOCUMENT COUNT FORM. Write the appropriate Program name in the School Name space. In the space labeled District/Program Number, write your school’s district number and Program Number.

12 Prepare Virtual School Pilot Program TO BE SCORED Answer Documents For Return ___14. Place all Connections Academy TO BE SCORED answer documents (all grade levels) in the envelope and seal it. ___15. Place all Florida Virtual Academy TO BE SCORED answer documents (all grade levels) in the envelope and seal it.

13 Packaging Writing + Test Materials You will be preparing the following types of boxes for return to the District Coordinator: Calibration TO BE SCORED Answer Documents – BLUE labels All TO BE SCORED Answer Documents – RED labels All NOT TO BE SCORED Materials – YELLOW labels District Coordinator ONLY Box *It is your responsibility to package the materials correctly for processing and score reporting. Mispacked materials delay the scoring and reporting of student results.

14 Calibration Schools BLUE If your school has been identified as a Calibration School, you will receive BLUE labels in your return materials kit. Follow these steps for preparing your materials for return to the District Coordinator: _Place the SSS Writing+ materials in the box(es). You may place materials for more than one grade level in a box. _Place the SSS Answer Document Return Summary for Calibration Materials on top of the documents in the first box of TO BE SCORED calibration materials. Seal the box(es). BLUE _Place the BLUE label anywhere on the top of each box that is being returned early. Make sure that packing tape does not cover the label. BLUE _Return the BLUE labeled box to the District Coordinator.

Home Ed., McKay Scholarship banded stacks containing, DOCUMENT COUNT FORM, and TO BE SCORED documents for each grade level/program 2. Answer Document Return Summary 1. Banded stacks containing your school’s DOCUMENT COUNT FORM, and TO BE SCORED documents by grade level

16 Sealing The Boxes Points To Remember 1. Make sure the packing tape does not cover the label. 2. Labels are scannable and should not be photocopied. 3. Do not place labels over the meeting point of the two flaps.

17 Labeling The Boxes Place the CORRECT COLORED LABEL on the top of each box.

18 Packing NOT TO BE SCORED Materials Yellow Labels 1. Place all NOT TO BE SCORED documents in the box(es). This would include: _Used and unused Test books _Unused non-preidentified answer documents _Preidentified answer documents _Answer documents with both DNS bubbles gridded 2. Seal the box(es). 3. Place the YELLOW LABEL on the top of the box 4. Return the box(es) to the District Coordinator

19 Preparing LARGE PRINT and BRAILLE Materials For Return Complete the following as soon as testing has ended: _Verify that your SSS FCAT Security Log has been completed correctly. _Remove stray planning sheets from the test documents. _Verify that each student grid sheet has been completed correctly or has a student label affixed. _Ensure that the student’s name and district/school number appear in the appropriate locations. _If the test is TO BE SCORED, verify that neither DNS bubble is gridded. If a session of the test is NOT TO BE SCORED, grid the DNS bubble

20 Preparing LARGE PRINT and BRAILLE Materials For Return (continued) Group each student’s SSS materials for return. Place the following documents for each student tested in a separate LARGE PRINT AND BRAILLE Materials RETURN ENVELOPE: _Completed Document Count Form _Large Print Writing+ SSS Test and Answer Book _Regular Print Writing+ SSS Answer Document _Braille Final Writing Sheets/Braille Paper containing student responses _Braille Writing+ Test Book _Regular Print Writing+ SSS Test BookNote: Although you may place documents for more than one SUBJECT in the same envelope, do not place materials for more than one STUDENT in the same envelope. DO NOT place NOT TO BE SCORED materials in a Large Print and Braille Materials Return Envelope.

21 Complete the LARGE PRINT and BRAILLE Materials Return Envelope _Complete an SSS Large Print and Braille Materials Return Envelope for each student according to the instructions on the envelope. Seal the envelope. _Return all Large Print and Braille materials in the original boxes in which they arrived. Seal the box(es). __Place a WHITE label anywhere on the top of each box. Labels should not be photocopied.

22 Packing District Coordinator Only Box(es) 1. Place all non-secure materials in the box(es). This would include: _Planning Sheets (used and unused) _Original Security Checklists _Original Pre-ID Roster _Original SSS FCAT Administration Record form _Original SSS FCAT Security Logs _Original Seating Charts _Large Print and Braille scripts, braille paper that does not contain student responses 2. Seal the box(es) and Write District Coordinator on the side of the box(es) 3. The District Coordinator will inform you when to return the boxes
