8 PFAC Vision and Mission
The Patient and Family Advisory Council Program is dedicated to strengthening collaboration between patients and family members and the health care team so as to enhance the ability of FBCH to deliver the highest standard of safe, comprehensive and compassionate health care. The Patient and Family Advisory Council will serve as the “voice” for patients who receive treatment at FBCH and clinics and their family members.

9 PFAC Purpose Strengthen communication and collaboration among patients, families, caregivers and the Fort Belvoir Community Hospital staff. Bring the patient /family perspective to hospital operations. Improve patient safety, quality, and delivery of care. Provide feedback about the needs and experiences of patients and families. Serve as a resource on a wide variety of issues, services and policies. Provide insight about how to humanize the healthcare experience. Provide patient and family insight into the planning for new services, programs, facilities, and facility renovations. Share our unique experiences and help us make a difference in the lives of other patients and their families. Develop community awareness of the PFAC through engaging the community.

3224 Unit: Password ____________ Nurse Station: ___ Patient Family Centered Care Our patient family centered approach is based on an understanding that family members are a primary source of strength and support. The healthcare team will be able to provide general information about the patient’s condition to family members who call in and have the password or who are with the patient at the bedside. Each room has a phone to allow the family to participate in healthcare team rounds, nurse shift change report, discharge planning and ask questions as appropriate. Our staff will make every effort to accommodate patient and family requests to participate.
