Head coach MUST wear 2006 GBA uniform shirt during game time, otherwise s(he) will not be permitted to coach the game. Assistant coach is not required to wear GBA uniform. A GBA polo shirt may be purchased for $20 Coaches

Players Must be in uniform, otherwise (s)he will not be allowed to play. Must use the same jersey number throughout the season. Numbers are assigned on the Final Roster.

Players must be 12 years of age by June 1, 2006 to be eligible to play. If a player turns 15 between January 1 and June 1, 2006 that player is ineligible. Registration form must be signed by Parent Custodian/ Guardian and submitted with a $5 registration fee and a copy of birth certificate by May 26th. Must have an OFFICIAL Dallas Park & Rec. ID at each game & practice; costs $5.

Players’ IDs MUST be presented at scorer’s table for check-in at the time of warm-ups. If a player’s birth certificate is tampered with, that player will be eliminated from the GBA. ATTENTION

All Coaches must complete and submit a Volunteer Application by today, May 22nd in order to coach by Week 1. Criminal Background Checks will be conducted on all applicants. Coaches are encouraged to fill out applications thoroughly and truthfully. ATTENTION

Verify each player’s age. If a player is ineligible to play based on his or her age, the player will not be allowed to participate in the league. Contact local recreation center and make arrangements to have players’ OFFICIAL Dallas Park & Rec. IDs made. Submit Final Roster, players’ registration, copy of birth certificate, with a color copy of player’s official IDs to site coordinators by May 26th. Note: Coaches may add/drop players thru June 8th & 9 th.

Complete and submit Protest Form to site coordinators no later than 15 minutes after the conclusion of your game. A $50 Protest Fee will be due at the scheduled appeal date with GBA committee. Note: If you do not receive a response from the GBA Committee by 5:00 PM the following Tuesday, please call the GBA Hotline at or

Submit completed team packets to GBA Liaisons by 1:00 PM on June 12 th. Verify players’ ID cards at scorer’s table before each game. Verify that all players and coaches are properly uniformed prior to each game.

Complete and submit protest forms by 12:00 PM the following Monday. Complete and submit Weekly Progress Report Forms by 10:00 PM. The GBA Committee Contact poster must be posted and visible at all times. All contact posters will be provided by GBA.

There should be 6 teams per site Maximum number of players per team: 12 Minimum number of players per team: 5 If teams cannot meet 5 player minimum, the game will be declared a forfeit. In the event neither team has 5 players, it will be declared a double forfeit, and both teams will receive a “Loss.”

Game time is forfeit time. All regular season games count toward the placing for site playoffs. All games will be played in two 15-minute halves. The clock will only stop with 1 minute left in the game, if the losing team is not down by more than 10 points.

The clock will stop on all time-outs. All time-outs will be 60 seconds. Each team will be allowed 2 time-outs per game and 1 time-out in overtime. Regulation time-outs will not carry over to overtime. Overtime will be 3 minutes long.

There will be no abusive language or fighting allowed. The officials are the only liaison between the teams and the site coordinator during game time.

If a player or coach is ejected, (s)he must sit out the remainder of that game and the following game. If a team has only one coach, and (s)he is ejected, then this game is declared a forfeit.

Eagle Award applicants must complete and submit an application by July 6 th & July 7 th See Attachment B The Eagle Award will be awarded to one boy and one girl based on outstanding participation in the following areas: Game Attendance Written Essay Community Service Attendance “Scoring On and Off the Court”

Participants will be assigned to clean-up the Turner Courts/ Rhoades Terrace Communities. All GBA participants will be required to participate. See Attachment C “Operation Clean-Sweep”, July 15

Guaranty Bank will present its Eagle Award to winners at the GBA Championship Game, August 5 th at Ellis Davis Athletic Arena. Eagle Award winners will receive a computer system and color printer.

Teams will be nominated based on the following: (up to 15 points) -Overall conduct (up to 15 points) (35 points) -Game Attendance (35 points) -Community Service Attendance (50 points)All players listed on the final roster (50 points) (25 points)Less than total number of players (25 points) Point System – Teams earn up to 100 points “Scoring On and Off the Court”
