Bromley’s Learning Disability Health Self Assessment Framework 2012 Action Plan By Noel Morrow

Bromley’s Big Learning Disability Self Assessment Action Plan People from health and the Council have agreed an Action Plan to improve health services for people with learning disabilities in Bromley. The Action Plan will be sent to the Co- Chairs and the Health and Wellbeing Board. There will be an Easy Read version of this Action Plan and Final report. It will be on the Internet. The Good Health Sub Group will check that the Action Plan is being carried out.

ACTION PLAN – ACCESS FOR HEALTH Registers do not hold the correct numbers of people with learning disabilties  Actions Checking GP, other health and Council registers. No Flagging System for Reasonable Adjustments  Actions New GP system to show someone needs extra support. This information will be sent when person referred to other health services.

ACTION PLAN – ACCESS FOR HEALTH  Number of Health Action Plans  Actions Collect numbers of Health Action Plans completed by GPs and see how good they are.

ACTION PLAN – KEEPING PEOPLE SAFE AND HAVING GOOD SERVICES Checking how people with learning disabilities are kept safe Actions Health services in the community will check that people using their services are kept safe and report this to groups like Bromley Safeguarding Adults Board.

ACTION PLAN – KEEPING PEOPLE SAFE AND HAVING GOOD SERVICES People who cannot make a decision about treatment or staying in hospital Actions All health providers will check that people who are not able to make a decision for themselves about treatment or the need to stay in hospital when they don’t want to are treated properly and report on this to groups like Bromley Clinical Commissioning Group.

Future Learning Disability Self Assessment Framework – Recent News The next Learning Disability Self Assessment Framework – Big Health Check - will have a new name. It will be called the Joint Health and Social Care Self Assessment. People are being consulted on how this will be done and what will be included. Next year 2013 – 2014
