1. Every year Northumberland Learning Disability Partnership Board asks people what they think about the health services they have used in the last year. Your views will be included in our local report for the National Commissioning for Quality Learning Disability Health Assessment Framework. This is so we can make sure health services know where they are doing well and where they need to do things better

2. We are asking people with learning disabilities living in Northumberland who have used health services between April 2011 and May 2012 to tell us what is good and what needs to be better. It’s really important that you tell us your views about services such as the doctors, dentist, opticians and chemist. There is a separate questionnaire if you have been to hospital as well. Please ask someone to help you if need to. You don’t have to answer all of them just the ones that you want to. 4. Who makes your appointment to see the Doctor? Does your GP know you have a learning disability? Can you have longer with your GP if you want? Can you have your appointment at a time that suits you? Does someone go with you to see the GP?

5. Does your GP explain things to you in a way you understand? Do you have a Health Check at the Doctor’s once a year? If you do what was it like for you ? Do you have a Health Action Plan? Has anything changed about your health in the last year?

6. Does someone help you get the health services you need? (A Health Facilitator) Does your Doctor have easy to understand health information? What could your Doctor or the practice do better?

7. Do you have a dentist? When was the last time you were seen by the dentist? If you needed treatment where did you go for it? Do you go to sleep so the dentist can look at your teeth? Other Health Services

8. Does the dentist tell you things in a way you understand? Can you get easy to understand information from the dentist? What could your dentist do better?

9. Have you had your eyes tested in the last 2 years? Where did you go to get your eyes tested? Do they help you to understand what is going on? Can you get easy to understand information from the optician about your eye test and what the test says ? Eye Care

10. What could your optician do better? Have you been asked to agree (consent) to medical treatment, like going to sleep to have a tooth out or having an operation? Was anyone else asked instead of you? Giving consent

11. How do you get your prescription? Does the chemist help you to understand about your medicines? If yes, what sort of help did they give? How can they help you better? Getting your prescription

12. Have you used services that check parts of your body (screening services) like breasts if you are a lady ? If you have did they send you easy to understand information about what was going to happen? Did they make you comfortable and explain everything when you went to be screened?
