The Transforming Care is central to the NHS 5 year forward view and is 1 of 4 key priority areas work along with cancer treatment, mental health and diabetes. Claire, Jenny & Robert visited the Head of the NHS in London, Simon Stevens and he listened to People from Hampshire about the sort of things they need for the future. The new name for the Winterbourne View Joint Improvement Programme (mistreatment of people with LD in hospitals a long way from home) LAs, CCGs and specialist commissioners to come together to build local community provision/services as better alternatives to hospital National ambition for discharge of patients and closure of all unnecessary inpatient beds March 2019 to get to a position of beds per 1 million population A new model of care and a broader range of personalised provision is needed Big area: 1 x County Council, 3 x Unitary Authorities, 11 x District & Borough Councils x Clinical Commissioning Groups, 235 practices, 1 x NHS England Team We are focussing on Children, Young People and Adults with a Learning Disability and/or autism

3 How we will work Be user-friendly and consider every person
Everyone is committed and will work to make it happen.  Ask about changes at the start Everyone has an equal right to have a say and be listened to. Use different ways for people to be involved that work for them.  Everyone can say what decisions they can agree to and what they can’t. Sometimes we may not all agree on a decision, we will note down when this happens.  New partners will be asked to sign up to our way of working. The steering group will check if all transforming care work has been done with people with lived experience We will be clear about which people are attending meetings and why We will find out the needs of people that are involved and support them to have their say
